Bèta-sitosterol

Wat is bèta-sitosterol?

Bèta-sitosterol is een plantensterol. Het heeft een chemische structuur die lijkt op cholesterol.

Belangrijkste kenmerken:

  • Beste bewijs voor gebruik bij patiënten met goedaardige prostaathyperplasie (BPH) of hypercholesterolemie. Er is geen sterk bewijs om het gebruik van bèta-sitosterol voor andere indicaties te ondersteunen.
  • Waarschijnlijk veilig bij oraal gebruik en mogelijk veilig bij topisch gebruik. Eventuele bijwerkingen kunnen zijn: misselijkheid, indigestie, gas, diarree, obstipatie of verminderde eetlust en andere.
  • Geen bekende belangrijke interacties.

De FDA (Food and Drugs Administration) keurt het gebruik goed van gezondheidsclaims voor voedingsmiddelen die bèta-sitosterol en andere plantensterolesters bevatten om het risico op coronaire hartziekte (CHD) te verminderen (6668). Dit is gebaseerd op de conclusie van de FDA dat bèta-sitosterol en andere plantensterolen het risico op coronaire hartziekte kunnen verminderen door het cholesterolgehalte in het bloed te verlagen. Hoewel er voldoende bewijs is dat bèta-sitosterol het cholesterolgehalte verlaagt, is er geen bewijs dat langdurig gebruik het risico op het ontwikkelen van CHD daadwerkelijk verlaagt. Verwar bèta-sitosterol niet met sitostanol, het verzadigde bèta-sitosterol-derivaat in Benecol (5340).

Wat is bèta-sitosterol?

Gebruik

Oraal
Oraal wordt bèta-sitosterol gebruikt voor coronaire hartziekte (CHD) en hypercholesterolemie, goedaardige prostaathyperplasie (BPH), prostatitis en galaandoeningen. Het wordt ook oraal gebruikt om de seksuele activiteit te versterken. Bèta-sitosterol wordt ook oraal gebruikt voor het versterken van het immuunsysteem, het voorkomen van immunosuppressie en ontsteking na deelname aan een marathon, verkoudheid, griep, Mexicaanse griep, HIV / AIDS, reumatoïde artritis (RA), tuberculose, psoriasis, allergieën, fibromyalgie, systemische lupus erythematosus (SLE), astma, androgene alopecia (mannelijke kaalheid), bronchitis, immuun trombocytopenie (ITP), migrainehoofdpijn, chronisch vermoeidheidssyndroom en menopauze symptomen.

Topisch
Topisch wordt bèta-sitosterol gebruikt voor de behandeling van wonden en brandwonden.

Voedingsmiddelen
In voedingsmiddelen wordt bèta-sitosterol toegevoegd aan sommige margarines die bedoeld zijn voor gebruik als onderdeel van een cholesterolverlagend dieet en om hartaandoeningen te voorkomen.

Gebruik

Werking

Bèta-sitosterol is een plantensterol. Het heeft een chemische structuur die lijkt op cholesterol, met een ethylgroep toegevoegd op positie 24. Het gemiddelde dieet levert ongeveer 175-200 mg bèta-sitosterol op, maar minder dan 5 procent wordt daadwerkelijk geabsorbeerd bij orale inname. Daarom veroorzaken deze plantensterolen niet enkele van de nadelige atherogene effecten die verband houden met cholesterol uit dierlijke producten. Bèta-sitosterol wordt vaak aan margarines toegevoegd als een cholesterolverlagend hulpmiddel. Vetten zijn nodig om plantensterolen op te lossen, dus margarines zijn een ideale drager. Fytosterol van sojabonen die 48 procent bèta-sitosterol, 26 procent campesterol en 21 procent stigmasterol bevatten, zijn toegevoegd aan rundergehakt (8528). Capsules die bèta-sitosterol bevatten, worden mogelijk niet goed in de darmen gedispergeerd, waardoor hun vermogen om de cholesterolopname te verminderen wordt beperkt (5814).

Hormonale effecten
Remming van 5-alpha-reductase, die de omzetting van testosteron in dihydrotestosteron (DHT) verhindert, kan bijdragen aan de activiteit van bèta-sitosterol bij androgenetische alopecia. Vermindering van de biologische beschikbaarheid van cholesterol door bèta-sitosterol kan ook de biosynthese van testosteron en DHT verminderen (15550). Er wordt gesuggereerd dat androgenetische alopecia een verhoogde gevoeligheid van de haarzakjes voor DHT met zich meebrengt, waardoor hun groeifase en grootte afneemt (15550).

Immunologische effecten
Er is voorlopig bewijs dat bèta-sitosterol immunostimulerende effecten kan hebben. Mengsels van beta-sitosterol en zijn glycoside sitosteroline lijken ook de proliferatieve reacties van T-cellen in vitro te versterken (3669,5342). Bèta-sitosterol kan ook de milde immuunsuppressie en ontsteking verminderen die wordt waargenomen bij marathonlopers na een race (5335).

Lipide verlagende effecten
Bèta-sitosterol remt de opname van cholesterol in de darmen door te strijden om de beperkte ruimte in gemengde micellen, waardoor de cholesterolopname met ongeveer 50 procent afneemt (5814). Omdat er echter minder cholesterol in het lichaam beschikbaar is, treden compensatiemechanismen in werking en verhogen deze de cholesterolsynthese in de lever (5814).

Prostaat effecten
Voor prostaathyperplasie suggereert dieronderzoek dat bèta-sitosterol de 5-alfa-reductase-activiteit zou kunnen remmen, hoewel finasteride (Proscar) krachtiger lijkt te zijn (11759). Laboratoriumonderzoek suggereert dat bèta-sitosterol antiproliferatieve effecten kan hebben op de prostaat, mogelijk door groeifactoren te remmen (11234). Bij dieren doet bèta-sitosterol de prostaat krimpen, maar dit is niet aangetoond bij mensen (11759).

Werking

Veiligheid

Waarschijnlijk veilig

Wanneer oraal en op de juiste manier gebruikt. Bèta-sitosterol is veilig gebruikt in onderzoeken die tot 18 maanden duurden (5327-5339,7198,10638).

Wanneer lokaal en op de juiste manier gebruikt. Bèta-sitosterol wordt tot 20 dagen met schijnbare veiligheid gebruikt (13526,35117,35118).

Zwangerschap en lactatie

Onvoldoende betrouwbare informatie beschikbaar; vermijd te gebruiken.

Effectiviteit

Goedaardige prostaathyperplasie
Inname van bèta-sitosterol 60-130 mg oraal in 2-3 verdeelde doseringen per dag verbetert de urinesymptomen significant, verhoogt de maximale urinestroom en vermindert het resterende urinevolume; het heeft echter geen invloed op de grootte van de prostaat (5327,5328,5329,7195,7198,34975). Hoewel enig voorlopig onderzoek geen voordeel van bèta-sitosterol op symptomen van BPH aantoont, beperken de gebruikte doseringen de korte duur van de behandeling, de kleine populatiegrootte en / of de ernst van de ziekte van de geïncludeerde patiënten de betrouwbaarheid van deze laatste resultaten (35124,96784).

Hypercholesterolemie
Het oraal innemen van bèta-sitosterol verlaagt het totale cholesterolgehalte en het LDL-cholesterol (low-density lipoprotein) significant, maar heeft weinig of geen effect op het high-density lipoproteïne (HDL) -cholesterolgehalte (6668,7195).

De effecten van combinatieformuleringen die bèta-sitosterol bevatten, zijn ook onderzocht bij patiënten met hypercholesterolemie. Uit voorlopig klinisch onderzoek blijkt dat het innemen van een combinatie van 8 gram soja-eiwit plus 2 gram bèta-sitosterol per dag gedurende 40 dagen, het LDL-cholesterol bescheiden verlaagt met 17-20 mg / dL in vergelijking met de uitgangswaarde bij patiënten met matige hypercholesterolemie. Deze combinatie heeft echter geen invloed op de triglyceriden- of HDL-cholesterolwaarden (35013,35039). Ander klinisch onderzoek toont aan dat het dagelijks innemen van een combinatie van 2,5 gram bèta-sitosterol en 8 gram cholestyramine (Questran, Bristol Myer Squibb) gedurende 12 weken het totale cholesterol met ongeveer 23 mg / dL en het LDL-cholesterol met 15 mg / dL verlaagt in vergelijking met baseline bij patiënten met hypercholesterolemie (35100). De combinatie is echter minder effectief in het verlagen van totaal- en LDL-cholesterol in vergelijking met 10 of 80 mg atorvastatine per dag (35100).

Familiaire hypercholesterolemie
Voorlopig klinisch onderzoek toont aan dat het gebruik van bèta-sitosterol gedurende 1-4 maanden tijdens het volgen van een cholesterolverlagend dieet, het totale cholesterol met 6 tot 13 procent en het LDL-cholesterol met 6 tot 20 procent verlaagt bij volwassen en pediatrische patiënten met familiaire hypercholesterolemie (3889, 5331,5332,5333,35067). Sommige onderzoeken tonen echter aan dat bèta-sitosterol ook de niveaus van high-density lipoproteïne (HDL) -cholesterol kan verlagen bij pediatrische patiënten met deze aandoening (5332). Ook lijkt bèta-sitosterol minder effectief te zijn in het verlagen van LDL-cholesterol in vergelijking met sitostanol bij pediatrische patiënten (3889). Bovendien lijkt bèta-sitosterol de triglycerideniveaus niet te verbeteren (5333).

Sommige onderzoeken hebben bèta-sitosterol vergeleken met cholesterolverlagende medicatie. Het innemen van 1 gram bèta-sitosterol driemaal daags in combinatie met 200 daags het fibraat geneesmiddel bezafibraat eenmaal daags, verlaagt het cholesterol even effectief als het nemen van 200 mg bezafibraat tweemaal daags bij kinderen met familiaire hypercholesterolemie.

Veiligheid

Interacties

Oraal wordt bèta-sitosterol meestal goed verdragen. Bij sommige patiënten kan het misselijkheid, indigestie, gasvorming, diarree, obstipatie of verminderde eetlust veroorzaken (5327,5328,5334). Het is ook in verband gebracht met erectiestoornissen en verlies van libido (5327,5329). Van bèta-sitosterol is ook gemeld dat het acne verergert (5331). Een combinatie van zaagpalmetto en bèta-sitosterol is in verband gebracht met enkelvoudige meldingen van verlies van eetlust, winderigheid, diarree en verergering van acne (15550).

Dermatologisch
Oraal kan bèta-sitosterol acne verergeren als het alleen of in combinatie met zaagbladpalm  wordt ingenomen (5331,15550).

Gastrointestinaal
Oraal kan bèta-sitosterol misselijkheid, indigestie, gasvorming, diarree of obstipatie veroorzaken (5327,5328). Bèta-sitosterol kan ook de eetlust verminderen (5334). Enkele meldingen van verminderde eetlust, winderigheid en diarree zijn gemeld bij patiënten die een combinatie van zaagpalmblad en bèta-sitosterol gebruikten (15550).

Neurologisch
Oraal kan beta-sitosterol erectiestoornissen en verlies van libido veroorzaken (5327,5329).

Medicijnen

Ezetimibe (Zetia)
Ezetimibe remt de absorptie van bèta-sitosterol in de darmen en verlaagt de plasmaconcentraties bij mensen met en zonder sitosterolemie. Bij mensen met milde tot matige hypercholesterolemie verlaagt ezetimibe de bèta-sitosterolspiegels met 41 procent (11985,11989).

Pravastatine (Pravachol)
Er zijn aanwijzingen dat pravastatine de bloedspiegels van bèta-sitosterol kan verlagen (3672). Theoretisch zou dit kunnen gebeuren met andere HMG-CoA-reductaseremmers (statines); simvastatine (Zocor) lijkt echter geen invloed te hebben op de bèta-sitosterolspiegels (3673). Andere statinegeneesmiddelen zijn onder meer atorvastatine (Lipitor), cerivastatine (Baycol), fluvastatine (Lescol) en lovastatine (Mevacor).

Carotenen
Bèta-sitosterol kan de opname en bloedspiegels van alfa- en bètacaroteen verminderen (5814).

Vitamine E
Bèta-sitosterol kan de opname en bloedspiegels van vitamine E verminderen (5814).

Ziekten

Sitosterolemie
Het gebruik van bèta-sitosterol kan sitosterolemie, een zeldzame erfelijke lipidenstapelingsziekte, verergeren (5326,10305). Mensen met deze aandoening hebben een verhoogde opname van cholesterol en bèta-sitosterol uit de voeding en een verminderde klaring van bèta-sitosterol. De totale lichaamsvoorraad van bèta-sitosterol is tot 17-voudig toegenomen. Verhoogde hepatische bèta-sitosterol-spiegels remmen op competitieve wijze het cholesterolkatabolisme, wat bijdraagt ​​aan hypercholesterolemie (3663). Patiënten met sitosterolemie zijn vatbaar voor premature coronaire hartziekte en xanthomen (3661,3662). Bèta-sitosterol en zijn glycoside sitosteroline zijn gecontra-indiceerd bij patiënten met sitosterolemie. Bèta-sitosterol lijkt echter veilig te zijn voor heterozygote dragers van sitosterolemie (10457).

Bèta-sitosterol wordt meestal gebruikt in dagdoseringen van 60-130 mg gedurende maximaal 18 maanden. Hogere doseringen van 3-4 gram worden gebruikt gedurende 1-3 maanden.
