L-Glutamine

Wat is L-Glutamine?

L-Glutamine is een aminozuur (een bouwsteen voor eiwitten) die vrij voorkomt in het lichaam. Het is het meest voorkomende aminozuur en is bij heel veel stofwisselingsprocessen betrokken (1). Wij maken zelf glutamine aan en wordt derhalve niet essentieel genoemd. L-Glutamine wordt voornamelijk gemaakt door de skeletspieren, de longen, de hersenen en het vetweefsel, en wordt door het bloed afgegeven aan de organen die het nodig hebben. Weefsel dat afhankelijk is van glutamine dat aangeleverd wordt via het bloed, zijn het immuunsysteem, het maag-darmkanaal, de nieren en lever (2). De helft van het circulerende glutamine wordt als energiesubstraat gebruikt en vervolgens geoxideerd. Ongeveer 15% wordt gebruikt voor gluconeogenese en 35% wordt weer hergebruikt voor eiwitsynthese. Glutamine fungeert als een transporteur voor stikstof en koolstof (3).

Hoewel glutamine traditioneel gezien wordt als niet essentieel (lichaam maakt het zelf aan), is glutamine essentieel voor het handhaven voor een gezonde darmfunctie, immuunrespons en aminozuur evenwicht. Daarbij wordt het in feite gezien als een essentieel aminozuur in periodes van stress of stress gerelateerd zoals bij een operatie of ziekte etc. (2, 4, 5). Glutamine speelt een rol in het stimuleren van immuuncellen, lymfocyten, cytokine productie en het doden van bacteriën door de neutrofylen en fagocytose activiteit (6, 7).

Omdat glutamine wordt gezien als niet-essentieel is er lange tijd geen belangstelling geweest om glutamine te suppleren. Echter is er steeds meer belangstelling ontstaan en wordt er nu onderzoek naar verricht. Glutamine wordt nu namelijk gebruikt om de bijwerkingen van medicinale behandelingen te elimineren. Bovendien wordt glutamine ingezet om sneller te herstellen na een beenmerg transplantatie of darmoperatie. Glutamine verhoogt het welzijn van mensen die lijden aan operaties en voorkomen van infecties bij voornamelijk zwaar zieke mensen. Onderzoek toont aan dat suppletie met L-glutamine bij ernstige ziekteprocessen beschouwd kan worden als een levensreddende interventie (8, 9, 10, 11). Bij deze mensen blijkt dat glutamine wel essentieel is omdat de eigen productie tekort kan schieten. Ook tijdens stress, vasten zware sportbeoefening, levercirrose of zware infecties kunnen toch gemakkelijk tekorten optreden. Sommige mensen gebruiken glutamine bij digestieve problemen zoals bij een lekkende darm, maagzweren, darmzweren en de ziekte van Crohn. Soms wordt glutamine ingezet bij depressie, stemmingswisselingen, irritaties, angst, slapeloosheid en voor het beter volhouden van sportprestaties.

Bij mensen met HIV (AIDS) helpt glutamine om gewichtsverlies te voorkomen.

Glutamine wordt ook gebruikt bij ADHD, cystinuria, sikkelcelanemie en als anti kater middel.

Wat is L-Glutamine?

Werking

L-glutamine is betrokken bij een goede darmfunctie, het immuunsysteem en andere essentiële processen in het lichaam voornamelijk ten tijde van stress. Dit inclusief de stress die ontstaat door ziekte of disfunctioneren van een orgaan. Bij een groot aantal stofwisselingsprocessen is L-glutamine betrokken zoals de zuur-base balans en de stofwisseling van eiwit, vet en koolhydraten. Glutamine kan een belangrijk nutriënt zijn voor de spieropbouw of wondheling. Maar ook speelt het een fundamentele rol bij de balans tussen katabolisme en anabolisme en de regulatie van de celvolume. Het is een belangrijk substraat voor het darmslijmvlies en dus ook het immuunsysteem. Het is tevens belangrijk als energie leverancier (stikstof en koolstof) voor veel cellen in het lichaam. Voor veel stoffen in het lichaam, zoals aminozuren en glucose, is ook glutamine nodig. Bijna alle aminozuren bezitten één aminogroep terwijl glutamine twee aminogroepen bezit. Om die reden speelt glutamine een centrale rol bij de stofwisseling van aminozuren. Alle andere aminozuren kunnen worden gemaakt door glutaminezuur en glutamine kan daarbij gemakkelijk in elkaar worden omgezet.

Stoffen zoals purines, nucleïnezuren, DNA-bouwstoffen, aminoglucose verbindingen maar ook hormonen en co-enzymen kunnen worden gesynthetiseerd.

Na een operatie of verwonding is er stikstof nodig om wonden te helen en om de vitale organen te laten functioneren. Ongeveer 1/3 van deze stikstof komt van glutamine. Wanneer er meer glutamine wordt gevraagd dan het lichaam kan maken kan er spierverval ontstaan, zoals bijvoorbeeld bij mensen met HIV/AIDS. Door het nemen van een glutamine supplement kan de hoeveelheid glutamine weer opgebouwd worden. Wanneer de darmflora beschadigd is, of een dysbiose heeft, kan er een enorm glutamine tekort ontstaan. Bij een beschadiging aan het darmslijmvlies kan toediening van L-glutamine in combinatie met N-Acetyl glucosamine een helende werking hebben (12, 13, 14).

Functies L-Glutamine

Bouwstof voor proteïnen
Als eerste functie kan natuurlijk genoemd worden dat het aminozuur glutamine als bouwsteen functioneert voor proteïnen.

Immuniteit
Zelfs in een periode dat er geen groot beroep wordt gedaan op ons immuunsysteem worden grote hoeveelheden glutamine gevraagd door bepaalde immuuncellen zoals lymfocyten en de macrofagen. Op het moment dat er een groot beroep wordt gedaan door een immuunrespons, de immuuncellen zich sterk moeten vermeerderen en wanneer er antistoffen moeten worden aangemaakt, zal het verbruik van de beschikbare glutamine enorm toenemen. Meestal is de hoeveelheid glutamine bij kortdurende stress situaties wel toereikend. Wanneer het beroep op glutamine langer voortduurt zal de productie tekort schieten met als gevolg immuunzwakte. Met name belangrijk voor het Common Mucosal Immune System (CMIS). Dit is ons eerstelijnsimmuunsysteem dat bestaat uit slijmvliezen van de luchtwegen, de geslachtsorganen, maar voornamelijk het darmkanaal. Het antilichaam IgA wordt hier geproduceerd met behulp van glutamine. Een tekort aan glutamine kan leiden tot een verminderde afweer tegen gifstoffen in de darm, maar ook in de luchtwegen. Infecties, zoals o.a. Candida kunnen hiervan het gevolg zijn.

Energievoorziening
Glutamine is een belangrijke energiebron voor de dunne darm en in de citroenzuurcyclus speelt glutamine eveneens een belangrijke rol. Glutamine wordt omgezet in de mitochondriën en is belangrijk voor de vorming van de brandstof voor de immuuncellen: lymfocyten, macrofagen en fibroblasten (1, 4, 15).

Glucosehuishouding
De afgifte van het darmhormoon glucagon-peptide-1 (GLP-1) is mede afhankelijk van glutamine. Deze belangrijke peptide remt glucagon en verhoogt de glucosegevoeligheid van de bètacellen in de alvleesklier waardoor de afgifte van insuline wordt bevordert en zodoende de bloedsuikerspiegel op peil blijft. Bovendien werkt het GLP-1 op de eetlust, deze wordt geremd. Voor het behandelen van obesitas, maar ook bij diabetes wordt er inmiddels veel onderzoek gedaan met veelbelovende resultaten.

Zuur base balans
Zowel onze nieren als onze longen spelen een cruciale rol in het in evenwicht houden van onze pH waarde in het bloed. Glutamine wordt door de nieren gebruikt door waterstofatomen te koppelen aan de NH3-groep van glutamine, deze worden uitgescheiden als ammoniumionen (NH4+). Op die manier is glutamine ook betrokken bij het in stand houden van een gezonde zuur-base balans.

Neurotransmittersynthese
Glutamine speelt een zeer belangrijke rol in het hersenmetabolisme. Het verwante glutaminezuur (glutamaat) is op zichzelf een stimulerende (exciterende) neurotransmitter. Om GABA te produceren dat weer een sederende (inhiberende) neurotransmitter is, hebben wij de stoffen vitamine B6, B12, mangaan en glutaminezuur nodig. GABA is het natuurlijke valium voor onze hersenen. De verhouding tussen GABA en glutamaat bepaalt de balans tussen stimulatie en sedering van ons zenuwstelsel.

Ontgiften
Glutathion is de belangrijkste stof in ons lichaam dat dient om te ontgiften. Glutamine speelt ook hier een sleutelrol want Glutathion is een tripeptide dat bestaat ui glutamine, cysteïne en glycine. Zowel cysteïne als glutamine zijn stoffen die in beperkte mate aanwezig kunnen zijn om glutathion te maken. Bij stress, ernstige ziekten of zware sportbeoefening kan er een glutamine tekort ontstaan en daardoor ook een glutathion tekort. Door suppletie van glutamine en ook N-Acetyl cysteïne kan glutathion synthese gestimuleerd worden. Hierdoor kan ontgiftingsprocessen die nodig zijn bij zware metalen belasting of andere gifstoffen plaatsvinden.

Bouwstenen DNA en RNA
Voor cellen die de eigenschap hebben snel te kunnen delen zoals die van het immuunsysteem en het darmslijmvlies speelt glutamine een belangrijke rol. Glutamine produceert pyrimidines, de bouwstenen van DNA en RNA.

Transport van stikstof en ammoniakafvoer
Ongeveer 1/3 van alle stikstof (N) die afkomstig is van eiwitafbraak wordt tussen organen getransporteerd in de vorm van glutamine. Het ammoniak dat vrijkomt bij het verbruik van glutamine wordt aan het bloed afgegeven. Vervolgens wordt het in de lever verwijderd om het teveel aan stikstof te lozen. Dit ammoniak kan worden gebruikt om glutaminezuur weer om te zetten in glutamine. Bij het niet goed functioneren van de lever zal het spierweefsel helpen bij detoxificatie van ammoniak. Wanneer dit ook tekort schiet dan kunnen toxische concentraties ammoniak ontstaan. Lactulose wordt soms gegeven om ammoniak in het lichaam te verlagen. In dat geval kan het geven van extra glutamine samen met lactulose gecontraïndiceerd zijn (16).

Werking

Veiligheid

Het gebruik van glutamine is veilig. Glutamine wordt goed verdragen in hoge doseringen tot zelfs 40 gram per dag (16, 17, 18, 19, 20, 21, 22, 24, 25, 27, 28, 30, 33, 45, 47, 48, 49, 50, 51, 52, 56, 57, 58, 59, 60, 61, 62, 65, 66, 67, 68, 69, 70, 71, 72, 73, 74, 75, 76, 77, 78, 79, 80, 81, 82, 83, 84).

Er is onvoldoende onderzoek bij zwangeren en zogende moeders waardoor het algemene advies dan is: vermijdt het gebruik.

Kinderen: Kinderen tussen 3 en 18 jaar zou geen hogere dosis gegeven moeten worden dan 0, 7 gram per kg lichaamsgewicht. Er is onvoldoendeonderzoek gedaan naar de veiligheid bij hoge dosering bij kinderen (85, 86, 87, 88, 89).

Contra-indicaties

Er zijn geen contra-indicaties bekend. Enige terughoudendheid kan worden betracht voor mensen die last hebben van het zogenaamde “Chinees restaurant”syndroom. Dit heeft te maken met de overgevoeligheid van glutamaat (E621) die rijkelijk wordt gebruikt als Ve-tsjin. Mensen die hiervoor overgevoelig zijn met hoofdpijn, duizelingen en hartkloppingen zouden eventueel ook overgevoelig kunnen zijn voor glutamine (15, 90).

Mensen die lijden aan mania, bij een bipolaire stoornis, kunnen beter het gebruik van glutamine mijden (90).

Ook diabetes patiënten zouden enigszins terughoudend kunnen zijn aangezien glutamine suppletie een effect heeft op de insulinesecretie.

Bij levercirrose: Glutamine zou mogelijk deze conditie erger kunnen maken. Mensen met levercirrose zouden suppletie met glutamine beter kunnen vermijden (91).

Veiligheid

Bijwerkingen

Voor zover bekend veroorzaakt L-glutamine bij gebruik van 5 gram per dag geen bijwerkingen (29, 30, 31, 32, 33, 67, 68, 69, 70, 92, 93, 94).

Interacties

Medicijnen

Interacties met reguliere of natuurgeneesmiddelen zijn mogelijk. Medicaties om aanvallen van epilepsie te voorkomen kunnen een interactie hebben met glutamine. Mogelijkerwijs zou glutamine (gemetaboliseerd  tot een exhiberende neurotransmitter) de effecten van deze medicijnen kunnen verminderen. Voorbeelden van deze medicijnen zijn phenobarbital, primidone (Mysoline), valproic acid (Depakene), gabapentin (Neurontin), carbamazepine (Tegretol), phenytoin (Dilantin) (43, 16, 95). Raadpleeg hiervoor een deskundige.

Laboratoriumtesten

Glutamine wordt gedeeltelijk gevormd tot ammonia en/of glutamaat. Bij hoge dosis glutamine inname kan het serum niveau van zowel glutamaat als ammonia verhoogd zijn (16, 85).

Interacties

Dosering

Meestal zal een dagdosering liggen rond de 4 tot 5 gram per dag. Bij voorkeur ingenomen een half uur voor de maaltijd of 2 uur erna verdeeld over de dag. De dosis kan worden verhoogd naar behoefte aangepast aan het klinisch beeld of op advies van een behandelaar (zie onderzoeken) (30, 29, 31, 32, 33, 94, 96, 65, 64, 96, 97). Glutamine liever niet mengen met een warme drank en niet vlak voor het slapen gaan.
