Vitamine B6

Wat is Vitamine B6?

Vitamine B6 is een lid van de vitamine B-familie. Het is aanwezig in veel voedingsmiddelen, waaronder granen, peulvruchten, groenten, lever, vlees en eieren. In het lichaam is vitamine B6 vereist voor het metabolisme van aminozuren. Het is ook betrokken bij het metabolisme van koolhydraten en lipiden. Verschillende vormen van vitamine B6 bestaan, waaronder pyridoxine, pyridoxal en pyridoxamine. Al deze vormen worden beschouwd als equivalent in het menselijk lichaam. Wanneer het wordt opgenomen in supplementen, wordt vitamine B6 vaak gebruikt in combinatie met andere B-vitamines in formuleringen van vitamine B-complexen. Vitamine B-complexen omvatten doorgaans vitamine B1 (thiamine), vitamine B2 (riboflavine), vitamine B3 (niacine / niacinamide), vitamine B5 (pantotheenzuur), vitamine B6 (pyridoxine), vitamine B12 (cyanocobalamine) en foliumzuur.

Wat is Vitamine B6?

Gebruik

Over het algemeen wordt vitamine B6 het meest gebruikt voor de behandeling van premenstrueel syndroom (PMS), dysmenorroe, vitamine B6-tekort, “ochtendmisselijkheid” tijdens de zwangerschap, misselijkheid en braken bij kinderen met enteritis, depressie geassocieerd met zwangerschap of gebruik van orale anticonceptiemiddellen, misselijkheid of hoofdpijn met gebruik van orale contraceptiva, depressie bij vrouwen na de menopauze en tandbederf tijdens de zwangerschap. Het wordt ook oraal gebruikt voor primaire homocystinurie, hyperhomocysteïnemie, de ziekte van Alzheimer, cognitieve stoornissen en het voorkomen van neuritis geassocieerd met isoniazid, vincristine of penicillamine. Vitamine B6 wordt ook oraal gebruikt voor het stimuleren van de immuniteit, spierkrampen diurese, conjunctivitis, cataracten, cystitis, primaire hyperoxalurie, het voorkomen van nierstenen, het voorkomen van fracturen, carpaaltunnelsyndroom ( CTS), nachtelijke krampen, artritis en allergieën. Het wordt gebruikt voor ADHD, het syndroom van Down, migraine, astma, op pyridoxine reagerende sideroblastische anemie, sikkelcelanemie, het voorkomen van bloedarmoede door pyridoxine-tekort, xanthureenzuurururie, primaire cystathioninurie, acne, eczeem, verschillende huid aandoeningen, stimuleren van de eetlust, hyperlipidemie, hypertriglyceridemie, hart- en vaatziekten, atherosclerose, beroerte, hypertensie, stralingsziekte, symptomen van de menopauze, onvruchtbaarheid, amenorroe-galactorroe-syndroom, postpartum-lactatie onderdrukken, slapeloosheid, en de verbetering van het herinneren van dromen. Vitamine B6 wordt ook oraal gebruikt voor duizeligheid, reisziekte, psychose, autisme, hyperkinese, acute chorea, chronische progressieve erfelijke chorea, tardieve dyskinesie, afwezigheid van (petit mal) aanvallen, febriele convulsies, atrofie van de choroïde en retina, atrofie van het vaatvlies, diabetes, diabetische neuropathie, leeftijdsgebonden maculaire degeneratie (AMD), alcoholintoxicatie, het voorkomen van leukopenie secundair aan mitomycine, reversie van procarbazine-neurotoxiciteit, het voorkomen van aanvallen geassocieerd met cycloserine, door fluorouracil geïnduceerde erythrodysesthesie en acute hydrazine-toxiciteit. Het wordt ook oraal gebruikt om de snelheid van het herstel van restenose na coronaire angioplastiek te verbeteren, de preventie van coronaire revascularisatie, inflammatoire darmziekte (IBD) en om de kans op de ziekte van Lyme te verlagen.

Gebruik

Werking

Vitamine B6 is nodig voor de metabolisme van aminozuren. Het is ook betrokken bij het metabolisme van koolhydraten en lipiden. In het lichaam wordt vitamine B6 omgezet in het co-enzym pyridoxaalfosfaat voor een breed scala aan metabole reacties. Deze reacties omvatten transaminatie van aminozuren, omzetting van tryptofaan in niacine, synthese van gamma-aminoboterzuur (GABA) in het CZS, metabolisme van serotonine, norepinefrine en dopamine, metabolisme van meervoudig onverzadigde vetzuren,  fosfolipiden en de synthese van de heemcomponent van hemoglobine. Een vitamine B6-tekort bij volwassenen heeft voornamelijk invloed op de perifere zenuwen, huid, slijmvliezen en het hematopoëtische systeem. Bij kinderen wordt ook het CZS getroffen. Een tekort kan voorkomen bij mensen met uremie, alcoholisme, cirrose, hyperthyreoïdie, malabsorptiesyndromen, congestief hartfalen (CHF) en bij degenen die bepaalde geneesmiddelen krijgen.

Anti-inflammatoire effecten
Verlaagde vitamine B6-concentraties zijn geassocieerd met verhoogde plasmaspiegels van C-reactief proteïne (CRP). CRP is een indicator van ontsteking die betrokken is bij verhoogde cardiovasculaire morbiditeit.

Antioxiderende effecten
Vitamine B6 (pyridoxine) heeft een aantal antioxiderende en vrije radicalen wegvangende activiteiten, wat de gunstige effecten op tardieve dyskinesie zou kunnen verklaren.

Bloeddrukverlagende effecten
Voorlopig klinisch en dieronderzoek duidt erop dat vitamine B6 (pyridoxine) de bloeddruk kan verlagen. Mogelijke werkingsmechanismen omvatten preventie van overproductie van endogene aldehyden en een verbeterd glucosemetabolisme.

Carpaal tunneleffecten
Er is gesuggereerd dat grote doses vitamine B6 nuttig kunnen zijn bij het carpaaltunnelsyndroom, gebaseerd op een waarneming van lage vitamine B6-weefselniveaus in autopsiemonsters van mensen met het syndroom. Weefselniveaus nemen echter af in dood of geïnfecteerd weefsel en het bestaan ​​van een vitamine B6-tekort tijdens het leven kan niet worden afgeleid uit deze waarnemingen.

Homocysteïne-verlagende effecten
Vitamine B6 is een co-factor voor enzymen die betrokken zijn bij een van de twee routes voor het metabolisme van homocysteïne. Remethylatie van homocysteïne naar methionine vereist folaat en vitamine B12. Foliumzuursupplementen verhogen de activiteit van dit pad en verlagen het homocysteïnegehalte bij vasten. Vitamine B6 is niet betrokken bij deze route en heeft daarom geen invloed op de homocysteïnegehalte in vasten. Trans-sulfuratie van homocysteïne om cystathionine vormen wordt gekatalyseerd door cystathion-beta-synthase, dat afhankelijk is van vitamine B6. Deze route is voornamelijk actief na inname van een methionine-lading (d.w.z. na een maaltijd). Tekorten aan vitamine B6 of cystathion-beta-synthase verminderen deze route en verhogen homocysteïnegehaltes na methionine toediening. Vitamine B6-supplementen kunnen homocysteïnegehalte na methioninebelasting verlagen. Bewijs suggereert dat verhoogde homocysteïnespiegels vasculaire endotheelcelbeschadiging kunnen veroorzaken, verminderde endotheelafhankelijke vasodilatatie vanwege verminderde stikstofmonoxide-activiteit, verhoogde oxidatie en arteriële depositie van lipoproteïnen met lage dichtheid (LDL), verhoogde adhesie van plaatjes en activering van de stollingscascade.

Immuun-effecten
Vitamine B6 wordt als nuttig beschouwd in de immuunrespons. Vitamine B6 is nodig voor de productie van antilichamen. Ook verhoogt bij kritisch zieke patiënten vitamine B6 het aantal T-lymfocyt-, T-helpers en T-suppressorcellen.

Oxalaat-uitscheidingseffecten
Voor nierstenen wordt vitamine B6 verondersteld heilzaam te zijn door de uitscheiding van oxalaat via de urine. De meeste nierstenen zijn samengesteld uit calciumoxalaat en oxalaat van hoge urine dat wordt in verband gebracht met de ontwikkeling en terugkeer van nierstenen. Bij patiënten met type I primaire hyperoxalurie lijkt vitamine B6 (pyridoxine) de vorming van steen te verminderen door de afbraak van de oxaalzuurprecursor, glyoxaalzuur, naar glycine te verschuiven in tegenstelling tot oxaalzuur.

Serotonine-effecten
Bij aandachtstekortstoornissen met hyperactiviteit (ADHD) kunnen sommige kinderen lage serotoninespiegels hebben. Dit is echter controversieel. Men denkt dat vitamine B6 de serotoninespiegel kan verhogen en de symptomen bij sommige kinderen met lage serotoninespiegels zou kunnen verbeteren. Sommige onderzoekers denken dat suppletie met vitamine B6 (pyridoxine) ook nuttig kan zijn voor sommige dysforische mentale toestanden omdat het de aanmaak van serotonine en GABA verhoogt, maar het bewijs is nog voorlopig.

Zwangerschap

Indien oraal en op de juiste manier gebruikt. Een speciaal product met aanhoudende afgifte dat dagelijks vitamine B6 (pyridoxine) 75 mg bevat, is FDA-goedgekeurd voor gebruik tijdens de zwangerschap. Vitamine B6 (pyridoxine) wordt ook beschouwd als een eerstelijnsbehandeling voor misselijkheid en braken tijdens de zwangerschap door het American College of Obstetrics and Gynecology. Het mag echter niet op lange termijn worden gebruikt of zonder medisch toezicht en nauwlettend toezicht.

Mogelijk onzuiver

Bij orale toediening in buitensporige doses. Er is enige bezorgdheid dat hoge doses maternale vitamine B6 (pyridoxine) neonatale aanvallen kunnen veroorzaken.

Lactatie

Bij orale toediening in doses die niet hoger zijn dan de aanbevolen dagelijkse hoeveelheid (ADH). De RDA bij vrouwen die borstvoeding geven is 2 mg per dag. Er is onvoldoende betrouwbare informatie beschikbaar over de veiligheid van vitamine B6 bij gebruik in hogere doses bij vrouwen die borstvoeding geven.

Effectiviteit

Erfelijke sideroblastaire bloedarmoede
Het innemen van vitamine B6 (pyridoxine) oraal is effectief voor de behandeling van erfelijke sideroblastische anemie. Het nemen van de actieve vorm van vitamine B6 (pyridoxal-5′-fosfaat) lijkt ook effectief te zijn in gevallen waarin de patiënt niet reageert op pyridoxine.

Pyridoxine-afhankelijke aanvallen
Pyridoxine-afhankelijke aanvallen zijn een zeldzame vorm van vuurvaste aanvallen bij pasgeborenen die niet reageren op anticonvulsiva. Deze aanvallen worden meestal gecontroleerd binnen enkele minuten na intraveneuze toediening van pyridoxine, een vorm van vitamine B6. Pyridoxine-afhankelijke aanvallen kunnen het gevolg zijn van genetische (autosomaal recessieve) afhankelijkheid van pyridoxine, of gebruikelijker het gevolg van het gebruik van hoge doses pyridoxine tijdens de zwangerschap.

Vitamine B6-tekort
Het oraal innemen van vitamine B6 is effectief voor het voorkomen en behandelen van een vitamine B6-tekort.

Hyperhomocysteïnemie
Het innemen van vitamine B6 (pyridoxine) oraal, alleen of in combinatie met foliumzuur, is effectief voor de behandeling van postprandiale hyperhomocysteïnemie. Het dagelijks innemen van pyridoxine 50-200 mg vermindert het postprandiale homocysteïnegehalte met 32% tot 35%.

Pyridoxine lijkt de homocysteïneconcentraties niet te verlagen tenzij foliumzuur en vitamine B12 op fysiologische niveaus zijn; het lijkt echter de homocysteïne-concentraties te verlagen bij patiënten die folaat en vitamine B12-uitputten wanneer ze worden gegeven met foliumzuur en vitamine B12-supplementen. Een combinatie van pyridoxine 100 mg en 0,5 mg foliumzuur per dag lijkt het homocysteïnegehalte met ongeveer 35% te verlagen. Een combinatie van deze vitamines wordt over het algemeen aanbevolen voor mensen met postprandiale hyperhomocysteïnemie. Pyridoxine in doses tot 200 mg per dag heeft geen effect op nuchtere homocysteïnespiegels en draagt ​​niet bij tot het effect van foliumzuur alleen op homocysteïnegehalten bij gezonde mensen.

Pyridoxine lijkt echter het homocysteïnegehalte te verlagen bij patiënten met terminale nierziekte (ESRD), niertransplantatiepatiënten en patiënten die algemene anesthesie krijgen met lachgas.

Hyperhomocysteïnemie is een risicofactor voor coronaire, cerebrale en perifere atherosclerose; terugkerende trombo-embolie; diepe veneuze trombose; hartinfarcten; en ischemische beroerte. Verhoogde homocysteïnespiegels kunnen echter een marker zijn in plaats van een oorzaak van vaatziekte. Een toename van 5 micromol in homocysteïne in plasma verhoogt het risico op cerebrovasculaire aandoeningen met 50% en de kans op coronaire hartziekten met 60% bij mannen en 80% bij vrouwen. Het is echter niet duidelijk of het verlagen van homocysteïnespiegels resulteert in verminderde cardiovasculaire morbiditeit en mortaliteit. Foliumzuur, vitamine B6 (pyridoxine) en vitamine B12-suppletie kunnen het totale homocysteïnegehalte verminderen van 13,4 tot 11,0 micromol per liter. Deze vermindering lijkt echter niet te helpen bij secundaire preventie van dood of myocardiale infarcten. In feite suggereert enig onderzoek langdurige suppletie, met pyridoxine plus foliumzuur en vitamine B12 voor secundaire preventie, een verhoogt risico op cardiovasculaire aandoeningen met 20% ondanks het verlagen van het homocysteïnegehalte met 30%.

Er is enige discussie over de vraag of suppletie met homocysteïne verlagende B-vitamines het risico op een beroerte kan verminderen. Een aantal klinische studies en meta-analyses tonen aan dat suppletie met foliumzuur, vitamine B6 en vitamine B12, alleen of in combinatie, het risico op beroerte bij patiënten met hart- en vaatziekten niet vermindert. Een meer recente meta-analyse van 10 klinische onderzoeken, waar meer dan 44.000 patiënten aan deelnamen, laat echter zien dat vitamine B-suppletie het relatieve risico op een beroerte op een bescheiden manier met 10% verlaagt in vergelijking met placebo bij risicopatiënten of bij patiënten met een voorgeschiedenis van hart- en vaatziekten. Deze meta-analyse verschilde van enkele van de eerdere analyses vanwege de opname van verschillende aanvullende klinische onderzoeken. Het is nog steeds onduidelijk welke specifieke combinatie van B-vitaminen optimaal is voor het verminderen van het risico op een beroerte en waarvan patiënten het meest zullen profiteren. Eén meta-analyse toont aan dat een combinatie van foliumzuur en vitamine B6 het risico op een beroerte effectiever verlaagt dan B-vitaminen die vitamine B12 bevatten. Een andere meta-analyse toont ook aan dat B-vitamines het risico op een beroerte verminderen bij een grotere omvang bij patiënten die geen vitamine B12 (cyanocobalamine) of lage doses cyanocobalamine krijgen in vergelijking met degenen die vitamine B-combinaties met hoge doses cyanocobalamine krijgen. Onderzoek toont ook aan dat het nemen van B-vitamines, waaronder vitamine B12, het risico op een beroerte niet vermindert bij patiënten met nierinsufficiëntie.

Veiligheid

Bijwerkingen

Oraal of door injectie kan vitamine B6 (pyridoxine) misselijkheid, braken, buikpijn, brandend maagzuur, gebrek aan eetlust, diarree, hoofdpijn, paresthesie, slaperigheid en verlaagde serum foliumzuurconcentraties veroorzaken. Het is ook in verband gebracht met meldingen van huid- en andere allergische reacties, pijnlijke borsten of vergroting en lichtgevoeligheid. Vitamine B6 (pyridoxine) kan sensorische neuropathie veroorzaken, die verband houdt met de dagelijkse dosis en de duur van de inname. Doseringen van meer dan 1000 mg per dag of totale doses van 1000 gram of meer vormen het grootste risico. Incidentieel kan sensorische neuropathie ook voorkomen bij lagere doses. Pyridoxaal-5-fosfaat (de biologisch actieve, co-enzymatische vorm van vitamine B6) heeft een betere biologische beschikbaarheid en is veiliger in grotere doses dan pyridoxine. Hooggedoseerde vitamine B6 (80 mg / dag als pyridoxine) en vitamine B12 (20 microgram / dag) kan resulteren in een intens erytheem met knobbeltjes, papels, puisten, mogelijk behandeling met systemische corticosteroïden en plaatselijke therapie kan verlichting brengen. Deze bijwerkingen zijn bij het gebruik van pyridoxaal-5-fosfaat echter nog nooit voorgekomen.

Interacties

Medicijnen

Amiodarone (Cordarone)
Wees voorzichtig met deze combinatie.

Voorlopig onderzoek suggereert dat vitamine B6 (pyridoxine) de door amiodaron veroorzaakte fotosensitiviteit kan verergeren. Ander onderzoek suggereert een beschermend effect.

Antihypertensieve drugs
Wees voorzichtig met deze combinatie.

Bewijs uit dierstudies en klinisch onderzoek suggereert dat vitamine B6 (pyridoxine) de systolische bloeddruk kan verlagen bij hypertensieve patiënten. Theoretisch kan vitamine B6 (pyridoxine) het risico op hypotensie verhogen wanneer het wordt gebruikt in combinatie met geneesmiddelen die de bloeddruk verlagen. Deze omvatten captopril (Capoten), enalapril (Vasotec), losartan (Cozaar), valsartan (Diovan), diltiazem (Cardizem), amlodipine (Norvasc), hydrochloorthiazide (HydroDiuril), furosemide (Lasix) en vele anderen.

Levodopa
Wees waakzaam met deze combinatie.

Vitamine B6 (pyridoxine) verbetert het metabolisme van levodopa, waardoor de effecten ervan tegen parkinsonisme worden verminderd. Deze interactie treedt echter niet op als carbidopa gelijktijdig met levodopa (Sinemet) wordt gebruikt. Daarom is het waarschijnlijk geen probleem bij de meeste mensen.

Phenobarbital (Luminal)
Wees voorzichtig met deze combinatie.

Voorlopig onderzoek suggereert dat vitamine B6 (pyridoxine) 200 mg per dag de plasmaspiegels van fenobarbital kan verlagen, mogelijk door het metabolisme te verhogen. Het is niet bekend of lagere doses enig effect hebben. Adviseer mensen die fenobarbital drugs gebruiken om hoge doses vitamine B6 te vermijden.

Phenytoin (Dilantin)
Wees voorzichtig met deze combinatie.

Voorlopige gegevens suggereren dat vitamine B6 (pyridoxine) 200 mg per dag de plasmaspiegels van fenytoïne kan verlagen, mogelijk door het metabolisme te verhogen. Het is niet bekend of lagere doses enig effect hebben. Adviseer mensen die fenytoïne gebruiken om hoge doses vitamine B6 (pyridoxine) te vermijden.

Sommige drugs kunnen de vitamine B6-niveaus beïnvloeden

Cycloserine (Seromycine)
Een supplement is nodig voor de meeste patiënten.

Cycloserine interageert met pyridoxaalfosfaat om inactieve hydrazonen te vormen, die pyridoxal-kinase remmen en vitamine B6-excretie in de urine verhogen. Dit verlies van vitamine B6 draagt ​​waarschijnlijk bij tot de neurotoxiciteit en toevallen die geassocieerd zijn met cycloserine. Vitamine B6-supplementen verminderen de incidentie van deze bijwerkingen. Het wordt aanbevolen dat mensen die cycloserine gebruiken ook vitamine B6 (pyridoxine) 150-300 mg / dag innemen.

Hydralazine (Apresoline)
Een supplement is bij sommige patiënten nodig.

Hydralazine kan vitamine B6-tekort veroorzaken bij sommige patiënten, waarschijnlijk door te reageren met pyridoxaalfosfaat om een ​​inactief hydrazine te vormen, dat wordt uitgescheiden. Geef vitamine B6-supplementen aan mensen die hydralazine nemen en vroege tekenen van perifere neuropathie ontwikkelen, zoals paresthesieën, gevoelloosheid en tintelingen.

Isoniazid (INH, Rifamate)
Een supplement is bij sommige patiënten nodig.

Isoniazid werkt in op vitamine B6 om een ​​inactief hydrazine te vormen, dat pyridoxal-kinase remt en de vitamine B6-uitscheiding in de urine verhoogt. Hierdoor worden lichaamstanks van vitamine B6 uitgeput en kan dit leiden tot perifere neuritis. Het komt zelden voor bij mensen die tot 5 mg / kg / dag isoniazide krijgen. Bij hogere doses isoniazide, kunt u overwegen vitamine B6-supplementen, 40 tot 250 mg / dag, vooral bij mensen met een voorgeschiedenis van convulsies, en bij degenen met andere risicofactoren voor neuropathieën, waaronder ondervoeding, uremie, diabetes, alcoholisme of leverziekte.

Penicillamine (Cuprimine)
Een supplement is nodig voor de meeste patiënten.

Penicillamine remt de activiteit van vitamine B6, mogelijk door een inactief complex te vormen met pyridoxaalfosfaat. Dit is in verband gebracht met perifere en optische neuropathie geassocieerd met het medicijn. Wanneer penicillamine wordt gebruikt voor de behandeling van de ziekte van Wilson, wordt aanbevolen dat patiënten vitamine B6 (pyridoxine) 25 mg / dag innemen. Er zijn geen richtlijnen vastgesteld voor het gebruik van vitamine B6 met penicillamine bij de behandeling van andere aandoeningen, hoewel supplementen van 50-150 mg / dag zijn gebruikt. Patiënten controleren op vroege tekenen van perifere neuropathie, zoals paresthesieën, gevoelloosheid en tintelingen, en vitamine B6-supplementen geven als deze zich voordoen.

Theophylline (Theochron, Theolair en anderen)
Een supplement is bij sommige patiënten nodig.

Theofylline remt pyridoxal-kinase, dat fosforylatie van vitamine B6 naar zijn actieve vorm, pyridoxal-5′-fosfaat, katalyseert. Mensen die met theofylline worden behandeld, hebben gewoonlijk verminderde plasma-pyridoxal-5′-fosfaatniveaus. De klinische betekenis hiervan is niet duidelijk, hoewel wordt gesuggereerd dat vitamine B6-deficiëntie bijdraagt ​​aan neurologische en CZS-bijwerkingen van theofylline, inclusief toevallen. Vitamine B6 (pyridoxine) supplementen van 10-300 mg / dag is geprobeerd bij mensen die theofylline innemen, maar de resultaten zijn tegenstrijdig. Plasmaspiegels worden soms weer normaal, maar het is niet duidelijk of er enig klinisch voordeel is.

Kruiden en supplementen

Kruiden en supplementen met hypoterenive effects
Vitamine B6 (pyridoxine) kan de bloeddruk verlagen. Theoretisch kan het nemen van vitamine B6 met andere kruiden en supplementen met hypotensieve effecten het risico op hypotensie verhogen. Sommige van deze kruiden en supplementen omvatten andrographis, caseïnepeptiden, kattenklauw, co-enzym Q10, visolie, L-arginine, lycium, brandnetel, theanine en andere.

Urobilinogeen
Vitamine B6 kan bij de spottest een vals-positief resultaat veroorzaken met de Ehrlich test.

Ziektes

Angioplastie
Een intraveneuze oplaaddosis foliumzuur, vitamine B6 en vitamine B12 gevolgd door orale toediening van foliumzuur 1,2 mcg plus vitamine B6 (pyridoxine) 48 mg en dagelijks ingenomen vitamine B12 60 mcg na coronaire stenting zou de restenose-snelheden kunnen verhogen. Vanwege het potentieel voor schade, moet deze combinatie van vitamines niet worden aanbevolen voor patiënten die coronaire stents krijgen.

Bariatrische chirurgie
Na een bariatrische operatie ontwikkelde een 50 jaar oude zwaarlijvige vrouw, die vitamine B6 300 mg dagelijks gedurende 6 maanden gebruikte, plasmaspiegels van pyridoxaalfosfaat die ten minste 61 keer hoger waren dan de normale niveaus. Het verhoogde pyridoxaalfosfaatgehalte was geassocieerd met misselijkheid, braken en bruine huid. Vanwege het risico op schade, moet aanvullende vitamine B6 in niveaus boven de voedingsnormen (DRI) niet worden aanbevolen na bariatrische chirurgie.

Oraal

Acne
Eén tot vier tabletten van een specifiek product (NicAzel, NicAzel, Elorac Inc., Vernon Hills, IL) met nicotinamide, azelaïnezuur, zink, vitamine B6 (pyridoxine), koper en foliumzuur, gedurende 8 weken is gebruikt.

Leeftijdsgebonden maculaire degeneratie (AMD)
Een combinatie van vitamine B6 (pyridoxine) 50 mg, vitamine B12 (cyanocobalamine) 1000 mcg en foliumzuur 2500 mcg dagelijks gedurende gemiddeld 7,3 jaar is gebruikt.

Angioplastiek
Een combinatie van B-vitamines, inclusief foliumzuur 1 mg, vitamine B12 400 mcg en vitamine B6 (pyridoxine) 10 mg per dag gedurende 6 maanden is gebruikt om de snelheid van restenose te verlagen bij patiënten die worden behandeld met ballonangioplastiek.

Atherosclerose
Een specifiek supplement (Kyolic, Total Heart Health, Formula 108, Wakunga) met verouderd knoflookextract 250 mg, vitamine B12 100 mcg, foliumzuur 300 mcg, vitamine B6 12,5 mg en L-arginine 100 mg, dagelijks genomen voor 12 maanden, is gebruikt.

Hart- en vaatziekte
Een combinatie van B-vitamines waaronder foliumzuur 2 mg, vitamine B6 (vorm niet gespecificeerd) 25 mg en vitamine B12 500 mcg, dagelijks genomen gedurende een mediane periode van 3-4 jaar is gebruikt.

Aan anticonceptie gerelateerde bijwerkingen
Vitamine B6 (pyridoxinehydrochloride) 25 mg per dag gedurende 6-12 maanden werd gebruikt.

Diabetes
Vitamine B6 (pyridoxine) 100 mg per dag gedurende 2 weken is gebruikt.

Diabetische neuropathie
Vitamine B6 (pyridoxine) 50 mg en thiamine 25 mg per dag is gebruikt. Ook is een combinatie van foliumzuur (L-methylfolaat) 3 mg/dag, vitamine B12 (methylcobalamine) 2 mg per dag en vitamine B6 (pyridoxal-5′-fosfaat) 35 mg per dag gedurende 24 weken gebruikt.

Dysmenorroe
Vitamine B6 (vorm niet gespecificeerd) 200 mg dagelijks gedurende vier menstruatiecycli werd gebruikt.

Erfelijke sideroblastische anemie
Aanvankelijk werd vitamine B6 (pyridoxine) 200-600 mg per dag gebruikt, met een afname tot 30-50 mg dagelijks na een adequate respons.

Hyperhomocysteïnemie
Voor postprandiale hyperhomocysteïnemie werd dagelijks vitamine B6 (pyridoxine) 50-200 mg alleen of als een combinatie van vitamine B6 (pyridoxine) 100 mg en foliumzuur 0,5 mg gebruikt.

Hypertensie
Vitamine B6 (pyridoxinehydrochloride) 5 mg / kg per dag gedurende 4 weken werd gebruikt.

Isoniazide-geïnduceerde neuropathie
Vitamine B6 (pyridoxine) 6 mg per dag gedurende een jaar is gebruikt.

Nierstenen (nephrolithiasis)
Vitamine B6 (pyridoxine) 25-500 mg dagelijks werd gebruikt om de oxalaatspiegels te verlagen bij patiënten met type I primaire hyperoxalurie.

Zwangerschap-geïnduceerde misselijkheid en braken
Vitamine B6 (pyridoxine) 10-25 mg drie of vier maal daags is gebruikt. Bij patiënten die niet reageren op alleen vitamine B6 (pyridoxine), wordt een combinatieproduct dat vitamine B6 (pyridoxine) plus doxylamine (Diclectin, Duchesnay Inc.) bevatte drie- tot viermaal daags aanbevolen. Als alternatief wordt een specifiek product (PremesisRx, KV Pharmaceuticals), dat 75 mg vitamine B6 met langdurige afgifte (pyridoxine) gecombineerd met vitamine B12 (cyanocobalamine) 12 mcg, foliumzuur 1 mg en calcium 200 mg bevat, dagelijks FDA-goedgekeurd receptproduct voor misselijkheid tijdens de zwangerschap.

Premenstrueel syndroom (PMS)
Vitamine B6 (pyridoxine) 50-100 mg per dag, alleen of met magnesium 200 mg, is gebruikt. Doses zo hoog als 500 mg per dag zijn ook gebruikt, maar dagelijkse doses van meer dan 100 mg lijken geen extra voordeel te hebben en kunnen het risico op bijwerkingen verhogen.

Tardieve dyskinesie
Vitamine B6 (vorm niet gespecificeerd) 100 mg per dag getitreerd wekelijks tot 400 mg per dag, gegeven in twee verdeelde doses, is gedurende 4 weken gebruikt.

Vitamine B6-tekort
Voor vitamine B6-tekort bij volwassenen is de typische dosis 2,5-25 mg dagelijks gedurende drie weken, daarna 1,5-2,5 mg per dag als onderhoudstherapie. Voor vitamine B6-tekort bij vrouwen die orale anticonceptiva gebruiken, is de dosis 25-30 mg per dag gebruikt.

Kinderen

Oraal

Acne
één tot vier tabletten van een specifiek product (NicAzel, NicAzel, Elorac Inc., Vernon Hills, IL) met nicotinamide, azelaïnezuur, zink, vitamine B6 (pyridoxine), koper en foliumzuur, gedurende 8 weken zijn gebruikt bij kinderen van minstens 12 jaar oud.

Hyperkinetisch cerebraal dysfunction syndroom
Vitamine B6 (pyridoxine) 300 mg per dag is gebruikt. Sommige kinderen kunnen echter grotere doseringen nodig hebben variërend van 500 mg tot 2 gram per dag.

Nierstenen (nefrolithiasis)
Vitamine B6 (pyridoxine) vanaf 10 mg / kg lichaamsgewicht per dag, verhoogd met 5 mg/kg elke 6 weken, tot een uiteindelijke dosering van 20 mg/kg per dag gedurende 24 weken is gebruikt bij kinderen van 5 jaar en ouder met type I primaire hyperoxalurie.

Intraveneus / Intramusculair

Pyridoxine-afhankelijke aanvallen
Vitamine B6 (pyridoxine) 10 tot 100 mg intraveneus of intramusculair wordt aanbevolen.

Vincristine-geïnduceerde neuropathie
Een case-rapport suggereert vitamine B6 (pyridoxine) 150 mg / m2 / dag kan nuttig zijn bij het verhelpen van de symptomen.

Standaardisatie en formulering

In de meeste klinische studies werd vitamine B6 alleen gegeven of als een combinatie met andere B-vitamines, meestal foliumzuur en vitamine B12. In de meeste proeven wordt vitamine B6 toegediend als pyridoxine of pyridoxal-5′-fosfaat. De opname van vitamine B6 uit voedsel of supplementen lijkt vergelijkbaar te zijn.
