Andrographis

Wat is andrographis?

Common Cold

Andograpgis extract in combinatie met Siberische Ginseng verbeteren de symptomen bij de behandeling van common cold wanneer er gestart wordt met behandelen binnen 72 uur bij de eerste symptomen. Na 2 dagen kan het verbeteren en normaal gesproken duurt de behandeling 4 tot 5 dagen voor een maximaal resultaat (34, 35, 36, 37, 38, 39). Voorlopig klinisch onderzoek suggereert dat ook het risico om ziek te worden door een kou met ongeveer 50% vermindert na behandeling met de kruiden. Meer onderzoek zal nog moeten worden gedaan (40).

Pharyngotonsilitis

Enkele klinische onderzoeken tonnen aan dat een hoge dosering Andrographis van 6 gram dagelijks in is te vergelijken met acetaminophen na 3 tot 7 dagen behandeling om de koorts te laten zakken en de keelpijn te laten verminderen die wordt veroorzaakt door pharyngotonsilitis (41).

Griep

Uit voorlopig onderzoek wordt aangetoond dat patiënten met de influenza die Andrographis extract gebruikt hebben in combinatie met andere kruiden zoals Siberische Ginseng eerder herstelden in vergelijking met patiënten die amantadine kregen. Ook complicaties die kunnen voorkomen bij influenza zoals sinusitis of bronchitis bleven uit (42).

Ulcerative colitis

Voorlopig klinisch onderzoek suggereert dat het nemen van Andrographis extract dagelijks en gedurende 8 wekende symptomen verminderen van een milde tot een gemiddelde ulcerative colitis in vergelijking met een placebo (43). Ook wordt aangetoond dat het nemen van Andrographis gedurende 8 weken net zo effectief is als met mesalamine om de symptomen van een patiënt met ulveratieve colitis te verminderen (44).

Wat is andrographis?

Gebruik

Andrographis wordt traditioneel gebruikt bij de behandeling van griep, ziekte van Lyme, ontsteking aan de slokdarm, allergieën, bijholteontstekingen, insectenbeten en bij HIV/aids. Andrographis wordt ook ingezet bij andere infectieziekten zoals malaria, syfilis, cholera, tonsillitis, longontsteking, tuberculose en zweren. Andere gebruiken zijn bij anorexia, arteriosclerose, slangenbeten, bronchitis, cachexie, preventie hart- en vaatziekten, diabetes, diarree, winderigheid, darmontstekingen, aambeien, toxische belasting van de lever en huidwonden. Andrographis wordt ook ingezet als een astringent, antiseptisch, antidote, anti-inflammatoir, antitrombotisch, laxatief en versterker.

Gebruik

Werking

De gebruikte delen van de plant zijn de bladeren en de wortel (1, 2, 12). De werkende stoffen die geïdentificeerd zijn: andrographolide, deoxyandrographioïde en andere diterpenes (3, 4, 5, 6). Ondanks dat Andrographis voor veel indicaties wordt gebruikt in de Ayurvedische en Traditionele Chinese geneeskunde is er moeilijk klinisch bewijs te vinden in de effectiviteit bij mensen behalve dan bij verkoudheid. Dit heeft te maken met het feit dat de Traditionele Chinese Geneeskunde de kruiden in een combinatie gebruiken met andere kruiden. Andrographis wordt beschouwd als een plant die immuno stimulerende eigenschappen bezit. Er is enig onderzoek dat aantoont dat de activiteit van antilichamen wordt verhoogt en fagocytose door de macrofagen worden gestimuleerd (7). Ook bleek uit onderzoek dat Andrographis de mastcellen stabiliseren en een antiallergische activiteit heeft (8, 9).

Andrographis is werkzaam bij infectieziekten, met name tegen de infecties rondworm (Ascaris lumbricoides), toxoplasma gondii, malaria, borrelia, en E. coli afscheiding (10, 11, 12, 13, 14, 15, 16, 17).

Mensen gebruiken Andrographis ook bij aids/HIV omdat enig constituents werd gevonden als anti HIV in vitro (18). Hoge dosis van de Andrographis kan ook de CD4 + cell counts verhogen bij de HIV patiënten. Andrographis wordt geacht de T-lymfocyten functie beter te stimuleren dan door direct inhibitie van virale replicatie (19).

Er is een beschrijving over de pijnstillende, antipyretische en anti-zweren effecten van Andrographis (12, 21).

Andrographis beschermt waarschijnlijk de lever tegen leve belastende medicijnen en chemicaliën, waarschijnlijk door de afgifte van gal, galzouten en galzuren (22, 23, 24, 25). De Andrographis heeft wat dit betreft een sterkere werking als Mariadistel (23, 25).

Andrographis heeft een gunstige werking bij hart- en vaatziekten. Enig bewijs is gevonden dat Andrographis de bloeddruk verlaagt, de platelet aggregation remt en de myocardiale ischemie vermindert (2, 4, 26, 27, 28, 29, 30).

Van Andrographis wordt vermeld dat het een abortieve activiteit bezit. Het mechanisme is nog onbekend (6). Ook kan het bij dieren de vruchtbaarheid zowel mannetjes als bij vrouwtjes negatief beïnvloeden, bij mensen wordt dit nog niet gevonden (31, 32, 33).

Werking

Veiligheid

De veiligheid is getest gedurende korte termijn inname en wordt als veilig beschouwd (41, 43, 44, 45). In combinatie met andere kruiden zoals Ginseng wordt de werking versterkt en is tevens veilig (Kan Jang Swedisch Herbal Instituut) (34, 35, 36, 38, 41, 42, 46). Een enkele studie duurde 3 maanden (40).

Bij kinderen is de veiligheid aangetoond bij kortdurend gebruik, ook in combinatie met de andere bovengenoemde kruiden (47, 48). Het wordt niet aangeraden om Andrographis te gebruiken tijdens de zwangerschap omdat de stimulerende werking mogelijk abortuele effecten zou kunnen hebben (12). Vermijdt het gebruik tijdens de zwangerschap

Lactatieperiode: Vanwege onvoldoende onderzoeken is het advies Andrographis niet te gebruiken tijdens het geven van borstvoeding

Veiligheid

Bijwerkingen

Over het algemeen wordt Andrographis goed getolereerd (39, 47, 48). Zelden maar mogelijke bijwerkingen kunnen zijn allergische reacties, uitslag, moeheid, hoofdpijn, diarree, misselijkheid, droge mond, metaal smaak, duizeligheid en een loopneus (35, 49, 50). Bij zeer hoge dosis 3 x daags 5 mg per kg lichaamsgewicht kan bovenvermelde bijwerkingen veroorzaken (49).

Bijwerkingen

Interacties

Kruiden en supplementen

Aangezien Andrographis een kruid is dat anticoagulant en antiplatelet activiteit heeft is het voorstelbaar dat er een versterkende werking gaat met kruiden die dit ook doen (zoals angelica, danshen, knoflook, ginkgo, panax ginseng, paardekastanje, curcuma en rode klaver), waardoor het mogelijk is dat dit het risico tot bloedingen verhoogt (1, 28).

Dit geldt ook voor de bloeddruk. Andrographis heeft een verlagend effect op de bloeddruk dus in combinatie met kruiden, zoals caseïne peptide, kattenklauw, conënzym Q10, visolie, L-Arginine, lycium fruit, theanine, die dit ook doen kan er theoretisch een onderdruk ontstaan (4, 26).

Medicijnen

Andrographis heeft een bloeddrukverlagend effect en is ook een anticoagulant en heeft een antiplatelet activiteit. Om dezelfde reden als bij de interactie met de bovenvermelde kruiden is het raadzaam alert te zijn bij het gebruik van medicijnen met dezelfde werking. Theoretisch is het mogelijk om dan het risico van bloedingen of een onderdruk te vergroten (1, 28).

Andrographis stimuleert de immuun activiteit en op die manier is het theoretisch mogelijk dat er een interactie ontstaat bij gebruik van immunologische onderdrukkende medicijnen zoals azathioprine (Imuran), basiliximab (Simuled), cyclosporine (Neoral, Sandimmune), daclizumab (Zenapax), muromonab-CD3 (OKT3, Orthocolone OKT3), mycophenolate (CellCept), tacrolimus (FK506, Prograf), sirolimus (Rapamune), prednisone (Deltasone, Orasone), en andere corticosteroïden (7).

Ziekten

Aangezien Andrographis een immuno stimulerend effect heeft bestaat er theoretisch een kans dat er een interactie is bij een auto immuunziekten. Er wordt geadviseerd op te passen door Andrographis te gebruiken bij SLE, MS en RA (7).

Interacties

Dosering

Andrographis wordt veelal in de TCM gedoseerd in combinatie met andere kruiden. Voor het bestrijden van de bacterie in deze combinatie is de dosering dan 2 gram per dag.

Bij behandeling van griep wordt gebruikt 3 x daags 400 mg (34, 35, 36, 37). Bij preventie kan men de helft inzetten (40). Voor verlagen van koorts of keelontsteking wordt dagelijks 3 gram gebruikt (41).
