PABA

Wat is PABA?

Para-aminobenzoëzuur (PABA) is een natuurlijk voorkomend, niet-eiwitaminozuur. PABA komt voor in voedingsmiddelen zoals lever, eieren, nier, tarwekiemen, zemelen, melk en yoghurt. Eerder werd gedacht dat het een B-vitamine was, maar het is nu bekend dat het noch een vitamine noch een essentiële voedingsstof is bij mensen (16, 266, 8954). Interessant is dat pathogene bacteriën PABA nodig hebben om foliumzuur te synthetiseren. Sulfonamide-antibiotica oefenen hun antibacteriële werking uit door de foliumzuursynthese uit PABA te remmen (16).

Wat is PABA?

Gebruik

Oraal, PABA wordt gebruikt voor vitiligo, pemphigus, dermatomyositis, morphea, scleroderma en de ziekte van Peyronie. PABA wordt ook oraal gebruikt om vrouwelijke onvruchtbaarheid, artritis, bloedarmoede, reumatische koorts, constipatie, systemische lupus erythematosus, lymfoblastoom cutis en hoofdpijn te behandelen. Het wordt ook gebruikt voor grijs haar, haaruitval en een verouderende huid.

Gebruik

Werking

Para-aminobenzoëzuur (PABA) is een onderdeel van het foliumzuurmolecuul en komt van nature voor in verschillende voedingsmiddelen, waaronder granen, eieren, melk en vlees (16, 266). PABA werd ooit beschouwd als een B-vitamine, maar wordt nu beschouwd als een niet-essentiële voedingsstof (16, 8954). Interessant is dat pathogene bacteriën PABA nodig hebben om foliumzuur te synthetiseren. Sulfonamide-antibiotica oefenen hun antibacteriële werking uit door de foliumzuursynthese uit PABA te remmen (16). Hoewel bloed, urine, zweet en ruggenmergvloeistof detecteerbare hoeveelheden PABA bevatten, is een tekort aan PABA niet beschreven bij mensen (266). Endogene PABA lijkt betrokken te zijn bij een aantal biologische processen, maar de precieze rol is niet bekend (266). De biologische beschikbaarheid van PABA kan afnemen met de leeftijd. Factoren zoals bacteriële overgroei als gevolg van chronische atrofische gastritis en verminderde secretie van maagzuur die kan optreden bij veroudering, kunnen de absorptie van PABA beperken (12460).

Werking

Veiligheid

Waarschijnlijk veilig

Bij lokaal en geschikt gebruik. PABA is door de FDA goedgekeurd voor actueel gebruik en er zijn geen meldingen van significante toxiciteit (266, 272).

Mogelijk veilig

Indien oraal en op de juiste manier gebruikt (10). PABA is een door de FDA goedgekeurd medicijn, maar enkele potentieel ernstige bijwerkingen zijn gemeld (10). Wanneer lokaal en op de juiste wijze op het oog aangebracht als een oogheelkundige oplossing van 0,007% (67957, 67963).

Mogelijk onveilig

Bij oraal gebruik in hoge doses. Doses van meer dan 12 gram per dag zijn in verband gebracht met leukopenie (1061).

Kinderen

Waarschijnlijk veilig

Bij lokaal en geschikt gebruik (266, 272). MOGELIJK VEILIG: indien oraal en op de juiste manier gebruikt (10). PABA is een door de FDA goedgekeurd medicijn voor gebruik bij kinderen.

Mogelijk onveilig

Bij oraal gebruik in hoge doses. Doses hoger dan 220 mg/kg/dag zijn in verband gebracht met fatale toxische effecten (1061).

Zwangerschap en lactatie

Waarschijnlijk veilig

Bij lokaal en geschikt gebruik (266, 272). Er is onvoldoende betrouwbare informatie beschikbaar over de veiligheid van het orale gebruik van PABA tijdens zwangerschap en borstvoeding; vermijd te gebruiken.

Effectiviteit

Effectief

Zonnebrand
PABA is een door de FDA goedgekeurde zonnebrand (266, 8952). PABA lijkt effectief te zijn tijdens zweten, maar niet wanneer de huid in water wordt ondergedompeld (272).

Mogelijk effectief

Ziekte van Peyronie
PABA is FDA-goedgekeurd voor gebruik bij patiënten met de ziekte van Peyronie. Klinisch bewijs toont aan dat het nemen van het specifieke PABA-product (POTABA, Glenwood LLC.) de kromming en verharding van de penis kan verminderen (1067, 8951, 67982, 68031).

Sclerodermie
PABA is door de FDA is goedgekeurd voor sclerodermie. Sommige retrospectieve studies suggereren dat het zou kunnen helpen voor sommige symptomen van sclerodermie, maar het meest overtuigende bewijs toont aan dat het niet helpt (1071, 1073, 1074, 1075).

Dermatomyositis
PABA is FDA-goedgekeurd voor gebruik bij dermatomyositis, maar er is beperkt bewijs om dit gebruik te ondersteunen (10).

Oog infectie
Voorlopig klinisch onderzoek suggereert dat bij de meeste patiënten het gebruik van een specifieke PABA 0,007% oftalmische oplossing (Actipol), gegeven als oogdruppels met of zonder gelijktijdige lokale injecties, effectief kan zijn bij de behandeling van herpes keratitis, een soort hoornvliesinfectie veroorzaakt door herpes simplex virus. Klinisch onderzoek suggereert dat deze PABA-oplossing even effectief of effectiever is dan acyclovir en effectiever dan idoxuridine (67957, 67963).

Morphea
PABA is FDA-goedgekeurd voor gebruik in morfea, maar er is beperkt bewijs om dit gebruik te ondersteunen (10).

Pemphigus
PABA is FDA-goedgekeurd voor gebruik bij pemphigus, maar er is beperkt bewijs om dit gebruik te ondersteunen (10).

Vitiligo
PABA is FDA-goedgekeurd voor gebruik bij vitiligo, maar er is beperkt bewijs om dit gebruik te ondersteunen (10).

Veiligheid

Bijwerkingen

Oraal lijkt PABA goed te worden getolereerd. De meest gemelde bijwerkingen zijn diarree, misselijkheid, braken, anorexia, dyspepsie, koorts en huiduitslag (1074). Allergische reacties waaronder koorts en huiduitslag zijn ook opgetreden (1074). In zeldzame gevallen is hepatotoxiciteit gemeld bij hoge doses (12-48 gram per dag) (1061, 1094). Hoge doses PABA (tot 48 gram per dag) kunnen ook een afname van het aantal witte bloedcellen veroorzaken (1061). PABA moet worden gestaakt als de patiënt niet kan eten (10). In één rapport stopte tot 25% van de patiënten met PABA vanwege intolerantie voor bijwerkingen (1074). Lokaal kan PABA contactdermatitis en soms paradoxale lichtgevoeligheid veroorzaken (272).

Interacties

Medicijnen

Cortisone (Cortisonacetaat)
PABA kan het metabolisme van cortison verminderen wanneer orale PABA en intramusculaire cortison gelijktijdig worden gegeven, waardoor mogelijk zowel de effecten als de bijwerkingen van cortison toenemen (4488). Aanpassing van de dosering van cortison kan nodig zijn.

Dapsone (Avlosulfon)
PABA kan de antibacteriële effecten van dapsone remmen; vermijd gelijktijdig gebruik (266).

Sulfonamide antibiotica
PABA remt de antimicrobiële activiteit van sulfonamide-antibiotica. Sulfonamide-antibiotica oefenen een antibacterieel effect uit door de foliumzuursynthese uit PABA competitief te remmen. Overmaat PABA kan het folaatafbrekende effect van de sulfonamiden overwinnen (10). Vermijd het gebruik van PABA gelijktijdig met sulfonamide-antibiotica. Sulfonamide-antibiotica omvatten sulfadiazine, sulfisoxazol (Gantrisin), sulfamethoxazol (Gantanol), sulfamethizol (Thiosulfil Forte), sulfasalazine (Azulfidine) en co-trimoxazol (trimethoprim-sulfamethoxazol, Bactrim, Septra).

Lab testen

Leukopenie
Het aantal witte bloedcellen kan tijdelijk worden verlaagd wanneer PABA wordt gestart, vooral bij hoge doses, maar normaliseert bij voortgezette toediening (10).

Levensfunctietests
Van PABA is gemeld dat het leverfunctietests verhoogt (1084). Monitor leverfunctietests bij aanvang, één maand na het initiëren van PABA-therapie, en daarna elke 3 tot 6 maanden (1084).

Ziektes

Renale ziekte
PABA wordt via de nier uitgescheiden en kan zich ophopen bij patiënten met nierdisfunctie; voorzichtig gebruiken (10). Dosisaanpassingen kunnen nodig zijn.

  • Morphea: de door de FDA goedgekeurde dosis voor volwassenen is 12 gram per dag in 4 tot 6 doses van het kaliumzout van PABA (10).
  • Pemphigus: de door de FDA goedgekeurde dosis voor volwassenen is 12 gram per dag in 4 tot 6 doses van het kaliumzout van PABA (10).
  • Vitiligo: de door de FDA goedgekeurde dosis voor volwassenen is 12 gram per dag in 4 tot 6 doses van het kaliumzout van PABA (10).

Topisch

  • Zonnebrand: PABA-zonnebrandmiddelen komen in concentraties van 1% tot 15% (272).
