Wat is Vitamine B2?

Riboflavine is een in water oplosbare B-vitamine (vitamine B2) die betrokken is bij vitale stofwisselingsprocessen in het lichaam en die nodig is voor normale celfunctie, groei en energieproductie. Kleine hoeveelheden riboflavine zijn aanwezig in de meeste dierlijke en plantaardige weefsels. Riboflavine wordt gevonden in veel voedingsmiddelen, waaronder melk, vlees, eieren, noten, verrijkt meel en groene groenten.

Wat is Vitamine B2?

Gebruik

Riboflavine wordt gebruikt voor het voorkomen van riboflavinedeficiëntie, het behandelen van ariboflavinose, meervoudig acylcoenzym A dehydrogenasedeficiëntie, rode bloedcelaplasie, migrainehoofdpijnen, acne, aangeboren methemoglobinemie, spierkrampen, malaria, beroerte, brandendevoetenyndroom en carpaaltunnelsyndroom. Het wordt ook gebruikt voor het voorkomen van oogvermoeidheid, staar en glaucoom. Riboflavine wordt gebruikt voor het verhogen van het energieniveau, het stimuleren van de werking van het immuunsysteem, het onderhouden van gezond haar, huid, slijmvliezen en nagels, voor het vertragen van veroudering, aften en geheugenverlies, waaronder de ziekte van Alzheimer. Het wordt ook gebruikt voor zweren, het stimuleren van atletische prestaties, het bevorderen van een gezonde voortplantingsfunctie, brandwonden, hypertensie, alcoholisme, leverziekte, sikkelcelziekte, multiple sclerose, de behandeling van melkzuuracidose geïnduceerd door nucleoside analoge reverse transcriptase-remmer (NRTI) -middelen, kwashiorkor, orale leukoplakie, verhoogde niveaus van homocysteïne en aandoeningen geassocieerd met zwangerschap, waaronder pre-eclampsie en zwangerschapgerelateerd ijzergebrek.

Gebruik

Werking

Riboflavine is een in water oplosbare vitamine die in het lichaam wordt omgezet in de cofactoren flavine-mononucleotide (FMN) en flavine-adenine-dinucleotide (FAD). Deze cofactoren hechten zich aan eiwitten die bekend staan ​​als flavoproteïnen. Dit activeert flavoproteïnen die een rol gaan spelen in vele enzymatische en metabolische processen in het lichaam. Riboflavine is nodig voor processen die te maken hebben met de activering van vitamine B6, de aanmaak van niacine, de activiteit van de bijnieren, de vorming van rode bloedcellen en het behoud van de integriteit van erytrocyten.

Boteffecten
Verhoogde inname van riboflavine verhoogde de botmineraaldichtheid bij personen met het TT-genotype (homozygote mutant) van MTHFR (71428).

Cardiovasculaire effecten
Riboflavine-afgeleide FAD is een cofactor voor het MTHFR-enzym dat betrokken is bij de remethylatie van homocysteïne naar methionine. Verbeterde riboflavine-status resulteert in homocysteïne-verlaging bij personen met het TT-genotype (homozygote mutant) van MTHFR. Er is geen homocysteïne-respons waargenomen bij patiënten uit de CC- of CT-groep (wildtype en heterozygote patiënten) met suboptimale riboflavine-status. Dit genetische polymorfisme is ook betrokken bij een verhoogde bloeddruk. Het TT-genotype resulteert in een MTHFR-enzym met verminderde activiteit, secundair aan het verlies van de riboflavine-cofactor, FAD. Dit effect wordt niet gezien bij patiënten met de CC- of CT-genotypen. 5-methyltetrahydrofolaat is een product van een MTHFR-gekatalyseerde reactie. Het reguleert stikstofmonoxide (NO). Het is ook nodig voor de remethylatie van homocysteïne naar methionine. Bij patiënten met het TT-allel nemen de concentraties van 5-methyltetrahydrofolaat af. Dit leidt tot verhoogde niveaus van homocysteïne in het plasma. Er wordt gespeculeerd dat suppletie met riboflavine het homocysteïnegehalte verlaagt en de 5-methyltetrahydrofolaatconcentraties kan herstellen en ook de afbraak van een co-factor van NO kan voorkomen, NO kan verhogen en de bloeddruk kan verlagen. Hoewel riboflavine verhoogde homocysteïnespiegels lijkt te verlagen bij degenen met het TT-genotype, is dit niet gecorreleerd aan een verlaging van de bloeddruk, maar dit wordt niet verondersteld het werkingsmechanisme te zijn vanwege de bloeddrukverlagende effecten.

Oefening prestatieverbeterende effecten
Bij humaan onderzoek resulteerde een beperkte inname van riboflavine in een significante afname van het aeroob vermogen, begin van de accumulatie van bloedlactaat en zuurstofverbruik. De rol van riboflavine-tekort in deze prestatiewijzigingen is echter niet duidelijk.

Metabolische effecten
Riboflavinedeficiëntie kan bijdragen aan lactaatacidose die kan optreden bij HIV-patiënten die stavudine (d4T, Zerit), zidovudine (AZT, Retrovir) en vergelijkbare nucleoside-analoog reverse transcriptase-remmer (NRTI) -middelen nemen (2024).

Effecten op het centrale zenuwstelsel (CZS)
Er is interesse in het gebruik van riboflavine voor multiple sclerose (MS). Demyelinisatie en ontsteking van het CZS doen zich voor bij patiënten met MS. Riboflavine is nodig voor het metabolisme van essentiële vetzuren. Bovendien is riboflavine betrokken bij de vorming van myeline in zowel het CZS als het perifere zenuwstelsel (PNS). Bij patiënten met een tekort aan riboflavine zijn componenten van zenuwen, waaronder myeline-lipiden en cerebrosiden, verminderd, en dit kan de hersenontwikkeling beïnvloeden. Homocysteïne kan een rol spelen bij MS. Het lijkt erop dat plasmaspiegels van homocysteïne worden beïnvloed door de plasmaconcentraties van riboflavine. Het vermoeden bestaat dat patiënten met MS hoge niveaus van homocysteïne hebben, wat kan leiden tot neurotoxische gebeurtenissen en cardiovasculaire disfunctie bij MS. Riboflavine kan het homocysteïnegehalte verlagen, maar in één onderzoek leek riboflavine geen effect te hebben in vergelijking met placebo, wat aangeeft dat de neurologische effecten ervan onafhankelijk kunnen zijn van homocysteïne.

Er is ook interesse in het gebruik van riboflavine bij patiënten die lijden aan migrainehoofdpijn. Er is gesuggereerd dat een verminderd gehalte aan voedingsstoffen, waaronder riboflavine, een rol kunnen spelen in de pathofysiologie van migraine. Voedingsstoffen, waaronder riboflavine, zijn noodzakelijk voor een normale mitochondriale functie. Riboflavine is een voorloper voor zowel FMN als FAD, die betrokken zijn bij elektronentransport in het mitochondriale membraan. Bij migraine is mitochondriale disfunctie aanwezig. Er wordt dus gespeculeerd dat suppletie met riboflavine de pijn kan verminderen die gepaard gaat met mogelijk hoofdpijn van migraine of voorkomt.

Oculaire effecten
FAD is een cofactor voor glutathione-reductaseactiviteit en de verhoogde glutathione waarden die door dit enzym worden geproduceerd, kunnen een rol spelen bij cataractpreventie. Glutathione-reductase vermindert geoxideerd glutathion tot zijn gereduceerde vorm, GSH. Van GSH wordt gedacht dat het de ooglens beschermt tegen oxidatieve schade. Patiënten met staar lijken lage GSH-waarden te hebben. Sommige experts theoretiseren dat lage niveaus van riboflavine ook geassocieerd zijn met staar, maar onderzoek is inconsistent. Sommige studies laten een tekort aan riboflavine zien en andere niet.

Werking

Veiligheid

Waarschijnlijk veilig

Indien oraal en geschikt gebruikt. Riboflavine is algemeen erkend als veilig (GRAS) in de VS. Riboflavine 400 mg per dag is gedurende maximaal 3 maanden ingenomen en 10 mg per dag is veilig gebruikt tot maximaal 6 maanden. Vanwege hoogwaardig veiligheidsonderzoek heeft de National Academy of Medicine (voorheen het Institute of Medicine) op dit moment geen aanvaardbare bovengrens van inname (UL) voor riboflavine vastgesteld, maar adviseert om overmatige inname te voorkomen.

Kinderen

Bij orale en adequate toediening van de aanbevolen dagelijkse hoeveelheid (ADH) van 0,3 mg per dag voor zuigelingen tot 6 maanden oud, 0,4 mg per dag voor baby’s van 6-12 maanden oud, 0,5 mg per dag voor kinderen 1-3 jaar oud, 0,6 mg per dag voor kinderen 4-8 jaar oud, 0,9 mg per dag voor kinderen van 9-13 jaar oud, 1,3 mg per dag voor mannen 14-18 jaar oud en 1,0 mg per dag voor vrouwen 14-18.

Zwangerschap en lactatie

Bij orale en gepaste toediening van de aanbevolen dagelijkse hoeveelheid (ADH) van 1,4 mg per dag voor zwangere vrouwen en 1,6 mg per dag voor vrouwen die borstvoeding geven.

Effectiviteit

Ariboflavinosis
Het nemen van oraal riboflavine gedurende maximaal een jaar voorkomt riboflavine-deficiëntie en verbetert ariboflavinose bij volwassenen en kinderen.

Staar
Een hoge inname van riboflavine via de voeding wordt geassocieerd met een verminderd risico op nucleaire cataract. Een hogere inname van riboflavine via de voeding lijkt ook geassocieerd te zijn met een verminderd risico op leeftijdsgebonden lensonacreatie. Mensen die een combinatie van riboflavine 3 mg plus niacine 40 mg per dag gebruiken, lijken ook een lager risico te hebben op het ontwikkelen van nucleaire cataracten in vergelijking met placebo.

Hyperhomocysteïnemie
Populatieonderzoek suggereert dat een slechte riboflavine-status geassocieerd wordt met hoge homocysteïnespiegels bij personen met het TT-genotype voor methyleentetrahydrofolaatreductase (MTHFR) -enzym. Inname van riboflavine 1,6 mg per dag gedurende 12 weken verhoogt de riboflavine-status en verlaagt het homocysteïnegehalte met 22% tot 40% bij personen met het TT-genotype van MTHFR in vergelijking met voorbehandeling. Suppletie met riboflavine lijkt echter niet het homocysteïnegehalte te verminderen bij personen met de CC- of CT-genotypen. Bij personen met hyperhomocysteïnemie door het gebruik van anti-epileptica, zorgt een dagelijks inname van riboflavine 75 mg, in combinatie met foliumzuur 0,4 mg plus pyridoxine 120 mg, voor een toename van het totale plasmaconcentratie van homocysteïne met 26% in vergelijking met de voorbehandeling.

Migraine
Het meeste klinische onderzoek dat alleen riboflavinesuppletie evalueert voor migraineprofylaxe bij volwassenen met migraine, toont aan dat de suppletie van 400 mg riboflavine dagelijks gedurende maximaal 3 maanden de migrainefrequentie kan verlagen in vergelijking met de niveaus van voorbehandeling. Significante verbetering in de frequentie, ernst en duur van migraine-aanvallen met riboflavine in vergelijking met placebo. In deze laatste studie hadden patiënten die riboflavine gebruikten gemiddeld twee migraineaanvallen per maand minder, 3,5 minder dagen hoofdpijn per maand en een gemiddelde hoofdpijnduur van migraine die 1,5 uur korter was in vergelijking met placebo. Riboflavine kan ook net zo effectief zijn voor het verminderen van de migraine-hoofdpijnfrequentie als de bètablokkers bisoprolol (Zebeta) en metoprolol (Lopressor).

Veiligheid

Bijwerkingen

In het algemeen wordt gedacht dat het beperkte vermogen van menselijke volwassenen om oraal toegediende riboflavine te absorberen, evenals de hoge uitscheiding ervan, het potentieel voor schade beperkt. In zeldzame gevallen zijn echter diarree, polyurie en chromaturie gerapporteerd na gebruik van oraal riboflavine.

Interacties

Medicijnen

Anticholinergische drugs
De snelheid van supplementaire riboflavine-absorptie verlaagd, maar de totale hoeveelheid van de geabsorbeerde vitamine verhoogd, door geneesmiddelen met anticholinergische effecten. Deze medicijnen verminderen de beweeglijkheid van de darm, waardoor de tijd dat de vitamine aanwezig is op absorptieplaatsen in de darm toeneemt. Dit is aangetoond met propantheline (Pro-Banthine), maar zou in theorie kunnen voorkomen bij elk geneesmiddel dat anticholinergische effecten heeft. De klinische betekenis is niet bekend.

Anticholinergica omvatten amantadine, atropine, belladonna-alkaloïden, fenothiazinen, scopolamine en tricyclische antidepressiva.

Phenobarbital (Luminal)
Enig bewijs bij dieren suggereert dat fenobarbital het metabolisme van riboflavine in de lever induceert. Niveaus van de actieve vorm van riboflavine, flavine-adenine-dinucleotide (FAD), verhogen in de lever en niveaus van een gehydroxyleerde metaboliet verhogen in de urine. De klinische betekenis hiervan is echter niet duidelijk. Ook suggereert dieronderzoek dat fenobarbital het gehalte aan flavine in de lever niet significant beïnvloedt.

Probenecid (Benemid)
Bij mensen vermindert probenecid de renale tubulaire secretie en de totale urinaire excretie van zowel supplementair als voedingsriboflavine. Het kan ook het actieve intestinale absorptiemechanisme voor riboflavine verminderen. Na enkelvoudige doses probenecide overheerst het effect op de renale excretie en zijn de plasmariboflavineniveaus verhoogd. Het effect van langdurige probenecide-therapie op de balans van riboflavine, en daarmee de klinische betekenis, is niet bekend.

Tetracycline antibiotica
In-vitro-gegevens suggereren dat riboflavine de potentie van tetracycline-antibiotica kan remmen. Het is niet duidelijk of dit effect klinisch significant is. In theorie kan het nemen van riboflavine met tetracycline-antibiotica de effecten van deze antibiotica verminderen.

Tetracycline-antibiotica omvatten demeclocycline (Declomycin), doxycycline (Vibramycin) en minocycline (Minocin).

Tricyclische antidepressanten (TCA’s)
Tricyclische antidepressiva hebben structurele overeenkomsten met riboflavine en kunnen interfereren met de omzetting ervan in de actieve vorm flavine adenine dinucleotide (FAD). In diermodellen verlagen zeer hoge doses amitriptyline en imipramine (50 mg / kg per dag) de FAD-niveaus in de lever, het hart en de hersenen. Lagere doses (10 mg / kg per dag) hebben alleen effect op het hart. Aangezien deze doses hoger zijn dan die therapeutisch worden gebruikt bij mensen, is het onwaarschijnlijk dat het effect klinisch significant is of dat supplementen noodzakelijk zijn.

Tricyclische antidepressiva omvatten amitriptyline (Elavil) of imipramine (Tofranil, Janimine) en anderen.

Kruiden en supplementen

Blond psyllium
Psyllium vermindert de absorptie van gesuppleerde riboflavine bij gezonde vrouwen.

Boron
Boorzuur kan complexeren met een portie riboflavine; dit effect verhoogt de wateroplosbaarheid van riboflavine. Als gevolg hiervan kan het nemen van boorzuur een significante uitscheiding van riboflavine veroorzaken binnen 24-48 uur na inname.

Folinezuur
Klinisch bewijs suggereert dat suppletie met foliumzuur 400 mcg dagelijks de riboflavinedeficiëntie verergert bij personen met het TT-genotype voor methyleentetrahydrofolaatreductase (MTHFR).

IJzer
Riboflavinesupplementen kunnen de hematologische respons op ijzersupplementen bij sommige mensen met bloedarmoede verbeteren. Aangenomen wordt dat riboflavine betrokken is bij het mobiliseren van ijzer uit de opslagvorm ferritine, voor heem- en globinesynthese, maar het lijkt de ijzerabsorptie niet significant te beïnvloeden. Het effect van riboflavine op het gebruik van ijzer is waarschijnlijk alleen significant bij mensen met riboflavinedeficiëntie.

Voedsel

Absorptie van riboflavinesupplementen kan verhoogd zijn bij inname met voedsel, omdat galzouten intestinale opname van riboflavine vergemakkelijken. Het absorptiemechanisme voor riboflavine is echter verzadigbaar; daarom is de klinische betekenis niet duidelijk.

Lab testen

Acetoacetaat decarboxylase
Riboflavine kan ten gevolge van enzymactivatie de serumacetoacetaat-decarboxylase-testresultaten onjuist verhogen.

Catecholaminen
Riboflavine kan ten onrechte de plasma- en urinemonluometrische catecholaminetestresultaten verhogen vanwege fluorescente metabolieten in plasma en urine.

Colorimetrische testen
Grote hoeveelheden riboflavine kunnen interfereren met urineonderzoek op basis van spectrometrie of kleurreacties. Grote hoeveelheden riboflavine veroorzaken felgele urine.

Urobilinogen
Riboflavine kan ten onrechte de plasma- en urinefluorometrische urobilinogeen-testresultaten verhogen vanwege fluorescerende metabolieten in plasma en urine.

Ziektes

Hepatitis, Cirrhose, Biliary obstructie
De absorptie van riboflavine is onder deze omstandigheden verminderd.

Oraal

De aanbevolen dagelijkse hoeveelheid (RDA) van riboflavine, zoals vastgesteld door de Voedsel en Voedingsraad van het National Institute for Medicine, voor volwassenen is 1,3 mg per dag voor mannen, 1,1 mg per dag voor vrouwen, 1,4 mg per dag voor zwangere vrouwen en 1,6 mg per dag voor vrouwen die borstvoeding geven. Er is geen gevestigde dagelijkse bovengrens inname (UL) voor riboflavine. Patiënten moeten worden geadviseerd om voldoende hoeveelheden riboflavine te blijven gebruiken om aan de dagelijkse behoeften te voldoen, maar niet in overmatige hoeveelheden.

Ariboflavinose
Riboflavine 5-30 mg per dag in verdeelde doses is gebruikt.

Staar
Een combinatie van riboflavine 3 mg plus niacine 40 mg dagelijks gedurende 5-6 jaar werd gebruikt.

Hyperhomocysteïnemie
Riboflavine 1,6 mg per dag gedurende 12 weken is gebruikt. Riboflavine 75 mg per dag in combinatie met foliumzuur 0,4 mg plus pyridoxine 120 mg gedurende 30 dagen werd gebruikt.

Migraine hoofdpijn
Riboflavine 25 mg of 400 mg per dag gedurende ten minste drie maanden is gebruikt. Een specifiek combinatieproduct (Dolovent; Linpharma Inc., Oldsmar, FL) dat een volledig dagelijkse dosis riboflavine 400 mg, magnesium 600 mg en co-enzym Q10 150 mg bevat, naast andere multivitaminen en sporenelementen, is gedurende 3 maanden gebruikt.

Kinderen

Oraal
