Wat is knoflook?

Knoflook is een wereldwijd geteeld kruid met een lange geschiedenis van culinair en medicinaal gebruik. Het is verwant aan uien, prei en bieslook.1,2 Er wordt gedacht dat knoflook inheems is in Siberië, maar meer dan 5000 jaar geleden werd verspreid naar andere delen van de wereld.1 Knoflook werd van oudsher gebruikt voor difterie, hoest, koorts, beten en steken, en aambeien.

Wat is knoflook?

Gebruik

Het toepasselijke deel van knoflook is de bol. De knoflookbol bevat het cysteïnesulfoxide. Veel van de farmacologische effecten van knoflook worden toegeschreven aan allicine, ajoene en andere alliïne, ook bekend als S-allyl-L-cysteïnesulfoxide.3 Wanneer de knoflookbol geplet, gemalen of gesneden wordt, wordt het alliïnebestanddeel omgezet in allicine (ook bekend als diallylthiosulfinaat) door het enzym alliinase.3,4,5 Verse knoflook bevat ongeveer 1% alliine. Eén milligram alliine wordt omgezet in 0,458 mg allicine.6 De hoeveelheid allicine in knoflookpreparaten is afhankelijk van de bereidingswijze. Processen waarbij maceratie van het teentje knoflook betrokken is, verhogen de activiteit van alliinase. Gevriesdroogde knoflook kan weinig of geen allicine bevatten. De leeftijd van knoflook beïnvloedt de hoeveelheid alliïne in het product. Knoflook wordt gerijpt om het gehalte aan andere zwavelverbindingen en de geur die vaak wordt geassocieerd met knoflook te verminderen. Het proces om geurloos gerijpt knoflookextract te produceren, vermindert het alliinegehalte tot slechts 3% van wat typisch in verse knoflook zit.6

Andere knoflookbestanddelen zijn onder meer S-propylcysteïnesulfoxide (PCSO) en S-methylcysteïne-sulfoxide (MCSO), die ook door alliinase kunnen worden omgezet in bestanddelen zoals allylmethaanthiosulfinaat, methylmethaanthiosulfinaat en andere thiosulfinaten. Vluchtige bestanddelen van knoflook zijn onder andere diallyldisulfide (DADS), dimethyltrisulfide (DATS) en zwaveldioxide.3

Van knoflookextracten is aangetoond dat ze bestanddelen bevatten zoals E-ajoene.3 Gerijpte knoflookextracten kunnen bestanddelen bevatten zoals S-allylcysteïne (SAC) en S-allylmercaptocysteïne (SAMC), terwijl knoflookolie bestanddelen kan bevatten zoals diallyldisulfide en dimethyltrisulfide.3,7 Warmte- en stoomdestillatie die wordt gebruikt om knoflookolie te produceren uit geplette knoflook, zet allicine om in allylsulfiden waarvan wordt aangenomen dat ze biologische activiteit hebben.4

Om de effectiviteit te verbeteren, kunnen knoflookpreparaten een maagsapresistente coating krijgen om de actieve bestanddelen te beschermen tegen degeneratie door maagzuur.8

Werking

Anthelminthische effecten

Bewijs uit dieronderzoek suggereert dat gehakte knoflook of waterig extract van knoflook de besmetting met Capillaria bij gewone karpers (Cyprinus carpio) kan verminderen.9 Het exacte mechanisme is onduidelijk en het is niet duidelijk of deze effecten waarneembaar zijn bij mensen.

Antibacteriële effecten

Verse knoflook heeft activiteit getoond tegen Escherichia coli, methicilline-resistente Staphylococcus aureus en Salmonella enteritidis; het is voorgesteld als voedseladditief om voedselvergiftiging te voorkomen.10 Naast verse knoflook is ook aangetoond dat ruw knoflookextract antimicrobiële activiteit heeft tegen Streptococcus mutans, wat suggereert dat het gunstig kan zijn voor het elimineren van bacteriën in de mond waarvan bekend is dat ze tandcariës veroorzaken.11,12 Ander in vitro onderzoek suggereert dat knoflookextract antibacteriële werking kan hebben tegen verschillende stammen van mycobacteriën, waaronder Mycobacterium tuberculosis, Mycobacterium avium-intracellulare en Mycobacterium kansasii, drie bacteriën die geassocieerd zijn met longinfecties.13,14,15,16 Knoflookextract lijkt echter geen additieve effecten te hebben bij gebruik in combinatie met antituberculosemedicijnen.13 Van knoflookolie is ook aangetoond dat het antimicrobiële activiteit heeft tegen Mycobacterium tuberculosis.16 De antibacteriële werking van knoflookolie wordt toegeschreven aan de allylzwavelverbindingen.17 Er is gesuggereerd dat de antimicrobiële activiteit van knoflookolie krachtiger is dan knoflookpoeder op basis van gewichtseenheden.18

De antimicrobiële effecten van knoflook zijn toegeschreven aan het allicinegehalte. Aangenomen wordt dat allicine de RNA-synthese remt en de DNA- en eiwitsynthese vermindert.19 Allicine en andere zwavelhoudende bestanddelen van knoflook kunnen ook de groei van bacteriën remmen door interactie met enzymen die nodig zijn voor groei en deze te inactiveren.20 Het knoflookbestanddeel ajoene bezit ook antibacteriële activiteit tegen zowel gram-positieve als -negatieve bacteriesoorten in vitro. De disulfidebinding in ajoene kan verantwoordelijk zijn voor deze effecten, maar het exacte werkingsmechanisme is onduidelijk.21

Antidiabetische effecten

Knoflook lijkt de bloedglucose te verlagen bij mensen met of zonder diabetes.22 Voorlopig klinisch onderzoek suggereert dat sommige verbindingen in knoflook, zoals S-methylcysteïnesulfoxide en S-allylcysteïnesulfoxide, enige antidiabetische werking kunnen hebben.23 In vitro bewijs suggereert dat S-allylcysteïnesulfoxide de insulinesecretie door bètacellen stimuleert.24 Ander in vitro bewijs suggereert dat verouderd knoflookextract de vorming van geavanceerde glycatie-eindproducten kan remmen, die geassocieerd zijn met diabetische complicaties.25

Schimmelwerende effecten

Aangenomen wordt dat de knoflookbestanddelen allicine en ajoene verantwoordelijk zijn voor de antischimmelactiviteit van knoflook tegen tinea-infecties.3,26 Gerijpt knoflookextract en zijn bestanddelen ajoene en allitriduim hebben antischimmelactiviteit aangetoond tegen Scedosporium prolificans, een schimmelsoort die immuungecompromitteerde patiënten kan treffen.27 Verse knoflook, gerijpt knoflookextract en het knoflookbestanddeel allicine hebben in het laboratorium antischimmelactiviteit tegen Candida albicans aangetoond.10,20,28 Bij mensen werd orale inname van 25 ml knoflookextract geassocieerd met anticandida-activiteit in serum tot 1 uur na inname.29 De antischimmelactiviteit van knoflook tegen candida-infecties wordt toegeschreven aan het vermogen van allicine om SIR2, een gen dat betrokken is bij de vorming van hyfen, omlaag te reguleren.30 Knoflook kan ook de synthese van lipiden door Candida albicans remmen, wat resulteert in cellekkage.20,31

Ontstekingsremmende effecten

Er zijn aanwijzingen bij mensen dat knoflook ontstekingsstoffen remt, waaronder C-reactief proteïne (CRP) en ontstekingscytokines, en de niveaus van homocysteïne verlaagt.32,33,34,35,36 De ontstekingsremmende effecten van knoflook kunnen een rol spelen bij de mogelijke klinische voordelen die verband houden met ziekten zoals artritis en hart- en vaatziekten.

Antilipemische effecten

Verschillende klinische onderzoeken hebben aangetoond dat knoflook de niveaus van totaal cholesterol, low-density lipoprotein (LDL) cholesterol, geoxideerd LDL-cholesterol, high-density lipoproteïne (HDL) cholesterol en triglyceriden kan verbeteren bij patiënten met hyperlipidemie of bij patiënten die hemodialyse ondergaan.37,38,39 Er zijn aanwijzingen dat verschillende knoflookbestanddelen krachtige remmers kunnen zijn van de hepatische cholesterolsynthese.40,41,42,43 Bij patiënten met hyperlipidemie kan knoflook het cholesterolgehalte verlagen door te werken als een HMG-CoA-reductaseremmer (statine).44,45,46,47,48 Knoflook kan ook de activiteit verminderen van enzymen die betrokken zijn bij de cholesterolsynthese, waaronder leverglucose-6-fosfaatdehydrogenase, appelzuurenzym en squaleenmono-oxygenase,42,49 evenals enzymen die de vetzuursynthese katalyseren, zoals vetzuursynthetase.46 Sommige laboratoriumgegevens suggereren echter dat knoflook ook de activiteit van cholesterol 7 alfa-hydroxylase, een enzym dat betrokken is bij de omzetting van cholesterol in galzuren, kan remmen of verminderen.46,47

Antioxiderende effecten

In verschillende laboratoriumonderzoeken vertoonden knoflook en zijn bestanddelen antioxiderende activiteit, waaronder het verhogen van de activiteiten van glutathionperoxidase, catalase en superoxidedismutase; verlaging van xanthine-oxidase-activiteit; en remming van lipideperoxidatie en prostaglandineproductie.50,51,52,53,54,55,56,57,58,59,60,61,62,63 Sommige laboratoriumgegevens suggereren dat de antioxidantcapaciteit van verse knoflook mogelijk groter is dan die van gedroogde knoflook.64 Bij mensen is het bewijs met betrekking tot het effect van knoflook op de serum-antioxidantcapaciteit of markers van oxidatie gemengd.65,66 Een meta-analyse van 12 gerandomiseerde, gecontroleerde klinische onderzoeken toont aan dat knoflook de totale antioxidantcapaciteit van het serum en de niveaus van superoxide-dismutase verhoogt, terwijl het serum malondialdehyde-niveaus verlaagt.67 Er was echter een aanzienlijke heterogeniteit, met knoflookdoses variërend van 80 – 4000 mg per dag, en een duur variërend van 2 – 24 weken. Onderwerpen waren onder meer gezonde mensen en mensen met verschillende ziektetoestanden.

Antiparasitaire effecten

Uit laboratoriumonderzoek blijkt dat knoflookolie een breedspectrumactiviteit heeft tegen Trypanosoma, Plasmodium, Giardia, Leishmania en Cochlospermum planchonii.68,69 Dieronderzoek suggereert dat het knoflookbestanddeel allicine infecties vermindert en de overleving vergroot van muizen die zijn blootgesteld aan Plasmodium, de parasitaire protozoa die malaria veroorzaakt.70 Bij dieren en in vitro vergroot knoflookextract de verzwelging door Leishmania en vernietigt het amastigoten door macrofagen.71,72 In vitro remt het knoflookbestanddeel ajoene enzymen die worden aangetroffen in Trypanosoma.73

Antivirale effecten

Voorlopig in vitro bewijs suggereert dat knoflookverbindingen, waaronder ajoene, allicine, allylmethylthiosulfinaat en methylallylthiosulfinaat, activiteit zouden kunnen hebben tegen virussen zoals cytomegalovirus, influenza B, herpes simplex virus type 1, herpes simplex virus type 2, parainfluenza virus type 3, vacciniavirus, vesiculair stomatitisvirus en humaan rhinovirus type 2.74,75,76,77,78 Effecten op atletische prestaties : Een enkele toediening van knoflook verhoogde het uithoudingsvermogen, waarschijnlijk als gevolg van een toename van de fibrinolytische activiteit in rusttoestand.79

Cardiovasculaire effecten

Menselijk onderzoek toont aan dat hooggedoseerd knoflookpoeder gedurende maximaal 4 jaar het arteriosclerotische en leeftijdsafhankelijke plaquevolume in zowel hals- als dijbeenslagaders vermindert.80 Voor leeftijdsgebonden vasculaire veranderingen en atherosclerose wordt aangenomen dat knoflook heilzaam is en vasculaire endotheelcellen beschermt tegen letsel door oxidatieve stress te verminderen, low-density lipoproteïne (LDL)-oxidatie te remmen en door antitrombotische effecten.81,82,83,84,85,86,87,88,89 Er zijn aanwijzingen dat LDL-oxidatie kan worden geremd door de organozwavelbestanddelen van knoflook, waaronder S-allylcysteïne, S-allylmercaptocysteïne, alliïne, allixine, N-acetyl-S-allylcysteïne en diallyldisulfide (DADS).86,90 Deze hypothese is echter betwist op basis van een proef van zes maanden bij vrijwilligers met matig hypercholesterolemie, die geen effecten van knoflooksuppletie op lipoproteïne-oxidatie konden aantonen.91 Ook lijkt knoflook de niveaus van lipoproteïne (a), een lipoproteïne vergelijkbaar met LDL, in menselijk plasma niet te verlagen.92

Naast het remmen van LDL-oxidatie suggereert laboratoriumonderzoek dat knoflookbestanddelen, waaronder diallylsulfide (DAS) en diallyltrisulfide (DATS), geoxideerde LDL-geïnduceerde expressie van E-selectine en vasculair celadhesiemolecuul (VCAM)-1 onderdrukken, waardoor de adhesie van monocyten aan endotheelcellen die optreedt in de vroege stadia van atherosclerose.93 Gerijpt knoflookextract lijkt geen invloed te hebben op de expressie van adhesiemoleculen noch op de hechting van leukocyten aan het oppervlak van het endotheel, hoewel het de vorming van vetstrepen en cholesterolophoping in de vaatwand lijkt te verminderen bij dieren die een cholesterolrijk dieet krijgen. suggereert een alternatieve methode voor bescherming tegen het ontstaan ​​van atherosclerose.94,95 Evenzo suggereert in vitro en ex vivo bewijs dat extract van knoflookpoeder de accumulatie van lipiden in atherosclerotische menselijke aortacellen kan verminderen door acyl-CoA:cholesterolacyltransferase-activiteit te remmen en cholesterylesterhydrolase te stimuleren.96,97 Er zijn aanwijzingen bij mensen dat knoflook ontstekingsstoffen remt, waaronder C-reactief proteïne (CRP) en ontstekingscytokines. Een verhoogd gehalte van deze verbindingen wordt in verband gebracht met een verhoogd risico op hart- en vaatziekten.32,33,34,35

Effecten op de bloedsomloop

In observationeel onderzoek verhoogde knoflooksuppletie de doorbloeding van de kalveren bij gezonde personen. De verbetering was geassocieerd met en mogelijk gemedieerd door verhoogde plasmaspiegels van interleukine-6 ​​(IL-6). In deze studie was verhoogde IL-6 onafhankelijk van de activering van de NO-route.153 In menselijk onderzoek verminderde knoflook de viscositeit van het bloed, wat het risico op hart- en vaatziekten en cerebrale vasculaire accidenten kan helpen verminderen.154

Stollingseffecten

Van knoflookpoeder en preparaten van gerijpte knoflook is aangetoond dat ze plaatjesaggregatieremmende eigenschappen hebben bij zowel patiënten met hart- en vaatziekten als bij gezonde vrijwilligers.155,156,157,158,159,160 Knoflook blijkt antitrombotische eigenschappen te hebben en kan de fibrinolytische activiteit verhogen, de aggregatie en adhesie van bloedplaatjes verminderen, de protrombinetijd (PT) verlengen en metabolische enzymen in bloedplaatjes remmen die verantwoordelijk zijn voor de omzetting van arachidonzuur in prostaglandinen en andere producten.3,7,157,161,162,163,164,165 Rauwe knoflook lijkt dosisafhankelijk cyclo-oxygenase van bloedplaatjes te remmen en serumtromboxaan B2 te verminderen.166,167,168,169,170 Antibloedplaatjesactiviteit kan worden toegeschreven aan knoflookbestanddelen, waaronder adenosine, ajoene, allicine en paraffinische polysulfiden.171,172 Rauwe knoflook lijkt krachtigere antibloedplaatjeseigenschappen te hebben dan gekookte knoflook.161,168,173,174,175 Knoflook pletten voor het koken kan een deel van het verlies van antibloedplaatjesactiviteit voorkomen.174 Knoflookolie lijkt de aggregatie van bloedplaatjes niet te beïnvloeden.176

Cytochroom P450-effecten

Er zijn aanwijzingen dat knoflook het cytochroom P450 (CYP450)-enzymsysteem kan beïnvloeden. In vitro bewijs suggereert dat knoflook verschillende CYP450-enzymen zou kunnen remmen, waaronder CYP2C9, CYP2C19 en CYP3A4.177 Maar klinisch onderzoek suggereert dat knoflookolie geen invloed heeft op CYP1A2, CYP2D6 of CYP3A4.17

Andere bevindingen geven aan dat knoflook de CYP2D6-activiteit licht kan remmen met ongeveer 9% bij mensen.179 Deze beperkte remming leidt waarschijnlijk niet tot klinisch significante toenames in geneesmiddelen die worden gemetaboliseerd door het CYP2D6-iso-enzym. Sommige onderzoekers vermoeden echter dat knoflooksupplementen die een aanzienlijke hoeveelheid allicine bevatten, het CYP3A4-iso-enzym kunnen induceren en klinisch significante dalingen kunnen veroorzaken in de niveaus van geneesmiddelen die door dit enzym worden gemetaboliseerd.180 In een kleine studie had het nemen van een specifiek knoflookproduct (GarliPure Maximum Allicin Formula, Natrolm Chatsworth) 600 mg tweemaal daags met 3.600 mcg allicine per dosis gedurende 12 opeenvolgende dagen echter geen significante invloed op de farmacokinetiek van docetaxel, een CYP3A4-substraat.181

Een knoflookextract dat alliïne en alliinase bevat, heeft geen invloed op de activiteit van CYP2D6 of CYP3A4.182 Klinisch onderzoek suggereert dat knoflookolie de CYP2E1-activiteit met 39% zou kunnen remmen.178 Het vermogen van knoflook om cytochroom P450-enzymen te induceren of te remmen kan afhangen van de aanwezigheid van andere bestanddelen in het preparaat dat wordt gebruikt.

Dermatologische effecten

In menselijk onderzoek is aangetoond dat knoflook wratten en likdoorns behandelt; het mechanisme is echter onduidelijk, maar kan verband houden met antivirale en versterkte immunologische effecten.183 Knoflook verminderde naar verluidt de cellulaire proliferatie in door virussen aangetaste cellen en oefende anti-DNA-activiteit uit.183 Het fibrinolytische effect van knoflook kan ervoor zorgen dat het omringende fibrineweefsel rond de likdoorncapsule lyseert en loslaat van het weefsel.183

Gastroprotectieve effecten

In vitro bewijs suggereert dat knoflookextract en commerciële knoflooktabletten antimicrobiële werking hebben tegen Helicobacter pylori, een veel voorkomende bacterie die geassocieerd wordt met maaginfecties of maagzweren.184,185 De anti-Helicobacter pylori-effecten van knoflook lijken het hoogst te zijn voor het knoflookbestanddeel allicine, gevolgd door onverdunde knoflookolie en vervolgens knoflookpoeder.186 De antibacteriële werking van knoflookproducten tegen H. pylori blijkt afhankelijk te zijn van het gehalte aan allicine.184 Voor knoflookbestanddelen lijkt de antimicrobiële activiteit toe te nemen met het aantal zwavelatomen in het bestanddeel.186

Vroeg bewijs suggereert een mogelijk beschermend effect van verouderde knoflook op intestinale toxiciteit geïnduceerd door methotrexaat (MTX) en fluorouracil (5FU).187,188 Voorlopig bewijs suggereert dat knoflook de orale opname van lood kan verstoren; de zwavelhoudende aminozuren zouden lood in het maagdarmkanaal kunnen cheleren.189 Samgyetang, een soep gemaakt van kip, jujube, Panax ginseng, knoflook en kastanjes, lijkt bescherming te bieden tegen experimenteel opgewekte maagzweren.190

Genito-urinaire effecten

Chronische inname van knoflook gedurende 70 dagen is in verband gebracht met onderdrukking van spermatogenese bij ratten.191

Hepatoprotectieve effecten

Er is enig voorlopig bewijs van hepatoprotectieve effecten van knoflook. Bij mensen verlaagt knoflooksuppletie de niveaus van aspartaataminotransferase (AST), maar niet alanineaminotransferase (ALT).192 In vitro is aangetoond dat gerijpt knoflookextract hepatoprotectieve effecten heeft.193 Volgens laboratoriumstudies zouden S-allylcysteïne en S-allylmercaptocysteïne de lever kunnen beschermen tegen paracetamol en koolstoftetrachloride, mogelijk door oxidatieve stress te verminderen.194 Ander voorlopig bewijs suggereert dat S-allylcysteïne de door doxorubicine geïnduceerde hart- en levertoxiciteit zou kunnen verbeteren.195

Immunologische effecten

Bewijs uit dieronderzoek suggereert dat knoflookolie 100-200 mg/kg om de dag gedurende 2 weken de proliferatiesnelheid van lymfocyten verhoogt en de productie van de cytokines interleukine 2 (IL-2), interferon-gamma (IFN-gamma), IL- 4, en IL-10 na stimulatie met concanavaline A.196 Bij lage doses lijkt knoflookolie de respons van T-cellen op de Th1-type cytokinen (bijv. IL-2 en INF-gamma) te versterken.196 Deze cytokines zijn in verband gebracht met een gunstige antitumorrespons.123 Bij hoge doses lijkt knoflookolie Th2-type cytokines (bijv. IL-4 en IL-10) te versterken bij hoge doses.196 Ander dieronderzoek suggereert dat een waterige knoflookoplossing van 600 mg/kg/4 ml per dag gedurende één maand de productie van INF-gamma verlaagt en de productie van IL-4 verhoogt in door fytohemagglutinine geactiveerde miltlymfocyten van ratten, wat suggereert dat knoflook een humor-immuunrespons kan bevorderen wanneer het wordt toegediend bij hogere doses.197

Sommige in vitro bewijzen suggereren dat extract van knoflookpoeder de door lipopolysaccharide geïnduceerde productie van IL-1beta en tumornecrosefactor (TNF)-alfa in menselijk volbloed vermindert. Knoflookpoeder-extract lijkt ook de activiteit van nucleaire factor (NF)-kB te verminderen, een transcriptiefactor die betrokken is bij ontstekingen geassocieerd met auto-immuunziekten zoals artritis en inflammatoire darmaandoeningen, evenals atherosclerose.198

Knoflook kan ook het aantal natuurlijke killercellen (NK) en de activiteit tegen tumorcellijnen verhogen.199,200 Hoewel rauw knoflooksap, verwarmd knoflooksap, gedehydrateerd knoflookpoeder en gerijpt knoflookextract allemaal de NK-celactiviteiten in tumordragende muizen lijken te verhogen, lijken alleen gerijpt knoflookextract en verwarmd knoflooksap de groei van tumorcellen te remmen.201

Knoflook lijkt ook de proliferatie van macrofagen en lymfocyten te stimuleren, wat het lichaam kan beschermen tegen immuniteitsonderdrukking veroorzaakt door chemotherapie of bestralingsbehandelingen.123 Dieronderzoek suggereert ook dat langdurige toediening van knoflook de aan leeftijd gerelateerde achteruitgang van de immuunfunctie kan verbeteren.104 Het knoflookbestanddeel alliine lijkt de fagocytische functie van perifere bloedleukocyten en monocyten te verhogen.202

Neurologische effecten

In dieronderzoek voorkwam verouderd knoflookextract verslechtering van op de hippocampus gebaseerde geheugentaken en had het antiamyloïdogene effecten.203,204 In vitro beschermde S-allyl-L-cysteïne tegen amyloïde beta- en tunicamycine-geïnduceerde neuronale dood.205 Het eiwit TRPA1 medieert de reactie op prikkelende irriterende stoffen die in knoflook worden aangetroffen en wordt voornamelijk aangetroffen in nociceptieve neuronen van perifere ganglia en in alle mechanosensorische epitheel van het binnenoor.206 In een transgeen muismodel met amyotrofische laterale sclerose vertoonde diallyltrisulfide (DATS) neuroprotectieve effecten.207 DATS heeft fase II-enzymen geactiveerd. SOD1-G93A transgene muizen kregen DATS oraal toegediend (80 mg/kg per dag). DATS verlengde de ziekteduur en levensduur met ongeveer een week, induceerde HO-1 en verminderde GFAP-expressie.

Oculaire effecten

Bij dieronderzoek verlaagde S-allylmercaptocysteïne, een van knoflook afgeleide verbinding, de intraoculaire druk; het kan de verhoging van ANP-niveaus inhouden.208

Stralingsbeschermende effecten

In vivo heeft het van knoflook afgeleide organozwavelbestanddeel allylmethylsulfide radioprotectieve effecten aangetoond, mogelijk door de door mitogeen geactiveerde proteïnekinasen (MAPK’s) en nucleaire factor-kappaB (NF-kappaB) -signaleringsroute neerwaarts te reguleren.209

Effecten op het skelet

In dieronderzoek onderdrukte knoflook botverlies als gevolg van oestrogeendeficiëntie .210 Bij ratten waarbij de eierstokken zijn verwijderd, kan knoflook de botresorptie remmen, zoals blijkt uit veranderingen in serum alkalische fosfatase-activiteit; serumtartraat-resistente zure fosfatase-activiteit; uitscheiding via de urine van calcium, fosfaat en hydroxyproline; en de calcium-tot-creatinine-ratio in de urine.211 In dieronderzoek voorkwam knoflook het verlies van botmineralen door de intestinale overdracht van calcium te bevorderen door de gedeeltelijke heropleving van de serumoestrogeentiter.212

Vasculaire effecten

Vasorelaxerende eigenschappen van knoflook zijn opgemerkt in meerdere preklinische onderzoeken.213,2014,215 Er is aangetoond dat de cutane microcirculatie bij mensen toeneemt na inname van 600 mg knoflook als gevolg van vasodilatatie van precapillaire arteriolen.216,217 Bij patiënten met hypertensie wordt aangenomen dat knoflook de bloeddruk verlaagt door relaxatie van gladde spieren en vasodilatatie te veroorzaken door de productie van endotheel afgeleide relaxatiefactor (EDRF, stikstofmonoxide) te activeren.218,219,220 Er is gesuggereerd dat allicine het bestanddeel van knoflook is dat verantwoordelijk is voor stikstofmonoxide-gemedieerde effecten.221 Knoflook kan ook leeftijdsgerelateerde toenames in aorta-stijfheid verminderen, wat de vaso-elasticiteit kan verbeteren en kan beschermen tegen hart- en vaatziekten.222

Effecten op gewichtsverlies

Uit dieronderzoek is gebleken dat de allyl-bevattende polysulfiden in knoflook verantwoordelijk zijn voor het verhogen van de thermogenese.223 Deze verbindingen versterken de thermogenese door de noradrenalinesecretie te verhogen via bèta-adrenerge stimulatie, verhogen de noradrenaline- en adrenaline-excretie en verminderen de accumulatie van lichaamsvet door het triglyceridenkatabolisme te verhogen door de verhoging van de thermogenese in bruin vetweefsel (BAT) en het verhogen van ontkoppelingseiwit (UCP).223

Wondgenezende effecten

In dieronderzoek werden wondgenezende effecten toegeschreven aan angiogene eigenschappen van verouderde knoflookoplossing.224 Het exacte mechanisme is niet bekend.

Werking

Veiligheid

Knoflook is oraal veilig in te nemen bij correct gebruik. Knoflook is veilig gebruikt in klinische studies die tot 7 jaar duurden zonder meldingen van enige significante toxiciteit.222,225,226,227,228,229,230,231,232,233,234,235,236,237

Ook is het mogelijk veilig bij plaatselijk gebruik. Knoflookbevattende gels, in vet oplosbare knoflookextracten, knoflookpasta’s en knoflookmondspoelingen zijn veilig gebruikt in klinisch onderzoek gedurende maximaal 3 maanden.238,239,240,241,242,243 Een vaginale crème met knoflook en tijm is 7 nachten veilig ’s nachts gebruikt.244

Bij plaatselijk gebruik (topicale toepassing) kan rauwe knoflook ernstige huidirritatie veroorzaken.

Bij zwangerschap is knoflook veilig in hoeveelheden die gewoonlijk in voedingsmiddelen worden aangetroffen.245  Het is mogelijk onveilig bij oraal gebruik in medicinale hoeveelheden. Van knoflook wordt gemeld dat het abortieve activiteit heeft.246 Een studie suggereert ook dat knoflookbestanddelen na een enkele dosis knoflook in het vruchtwater terechtkomen.247 Er zijn echter geen gepubliceerde meldingen dat knoflook een negatieve invloed heeft op de zwangerschap. In klinisch onderzoek werd dagelijks 800 mg knoflook gebruikt tijdens het derde trimester van de zwangerschap zonder bijwerkingen.248,249 Er is onvoldoende betrouwbare informatie beschikbaar over de veiligheid van topische knoflook tijdens de zwangerschap.

Hetzelfde geldt voor inname van knoflook tijdens borstvoeding. Verschillende kleine onderzoeken suggereren dat knoflookbestanddelen worden uitgescheiden in de moedermelk en dat zuigelingen die borstvoeding geven van moeders die knoflook consumeren, vatbaar zijn voor langdurige borstvoeding.245,250,251 Er is onvoldoende betrouwbare informatie beschikbaar over de veiligheid van topische knoflook tijdens borstvoeding.

Voor kinderen is knoflook in medicinale hoeveelheden veilig bij oraal en correct gebruik gedurende maximaal 8 weken. Knoflookextract 300 mg driemaal daags is met schijnbare veiligheid gedurende maximaal 8 weken gebruikt bij kinderen van 8-18 jaar.252 Er is onvoldoende betrouwbare informatie beschikbaar over de veiligheid van knoflook bij langdurig gebruik of in hogere doseringen.

Interacties

Medicijnen

Anticoagulantia: knoflook kan plaatjesaggregatieremmende effecten hebben en kan het risico op bloedingen verhogen als het wordt gebruikt met anticoagulantia of plaatjesaggregatieremmers. Rauwe knoflook en verschillende knoflookextracten hebben antibloedplaatjesactiviteit en kunnen de protrombinetijd verhogen.7,155,157,159,160,254

Antidiabetica: het nemen van knoflook met antidiabetica zou het risico op hypoglykemie kunnen verhogen. Klinisch onderzoek suggereert dat knoflook en knoflookextract de bloedglucosewaarden verlagen bij gezonde personen en personen met diabetes.22,37

Antihypertensiva: het nemen van knoflook met antihypertensiva zou het risico op hypotensie kunnen verhogen. In menselijk onderzoek hebben zowel knoflook als knoflookextracten bloeddrukverlagende effecten aangetoond.255,256,257,258,259,260,261

Atazanavir (Reyataz): knoflook zou mogelijk de niveaus en effecten van atazanavir kunnen verminderen.

In een casusrapport ontwikkelde een patiënt die driemaal per week zes geroerbakte knoflooktenen consumeerde, suboptimale atazanavir-niveaus en verhoogde HIV-virale belasting. Hoewel de exacte oorzaak van deze interactie onduidelijk is, wordt er gespeculeerd dat knoflook de intestinale absorptie van atazanavir zou kunnen verminderen of het metabolisme zou kunnen verhogen door cytochroom P450 3A4 (CYP3A4) te induceren.262 Totdat er meer bekend is, adviseren patiënten om geen grote hoeveelheden knoflook te consumeren tijdens het gebruik van atazanavir.

Cytochroom P450 2E1 (CYP2E1) substraten: knoflook kan de niveaus van geneesmiddelen die door CYP2E1 worden gemetaboliseerd, verhogen. Klinisch onderzoek suggereert dat knoflookolie de activiteit van CYP2E1 met 39% kan remmen.178 Gebruik knoflookolie voorzichtig bij patiënten die geneesmiddelen gebruiken die door deze enzymen worden gemetaboliseerd.

Cytochroom P450 3A4 (CYP3A4) substraten:  theoretisch gezien kunnen knoflookproducten die allicine bevatten CYP3A4 in de darm induceren en CYP3A4 in de lever remmen. Dit kan de niveaus van geneesmiddelen die door CYP3A4 worden gemetaboliseerd, verhogen of verlagen.

Isoniazide: knoflook zou de hoeveelheid isoniazide kunnen verlagen. Dieronderzoek suggereert dat een waterig extract van knoflook de isoniazidespiegels met ongeveer 65% verlaagt. Knoflook verminderde de maximale concentratie (Cmax) en oppervlakte onder de curve (AUC), maar niet de halfwaardetijd van isoniazide. Dit suggereert dat knoflookextract de opname van isoniazide door het darmslijmvlies zou kunnen remmen;263 het exacte mechanisme van deze potentiële interactie is echter niet bekend.

Proteaseremmers (PI’s): knoflookproducten die allicine bevatten zouden mogelijk de niveaus van PI’s kunnen verlagen. Proteaseremmers worden gemetaboliseerd door cytochroom P450 3A4 (CYP3A4) iso-enzymen. Er bestaat bezorgdheid dat knoflookproducten die allicine bevatten, CYP3A4 in de darm kunnen induceren, waardoor de plasmaspiegels van proteaseremmers worden verlaagd. Dit is voornamelijk gebaseerd op een onderzoek dat aantoont dat het tweemaal daags gedurende 3 dagen innemen van een specifiek knoflookproduct (GarliPure Maximum Allicin Formula, Natrol Inc.) de niveaus van saquinavir, een PI, met ongeveer 50% verlaagt. Er wordt gespeculeerd dat het allicinebestanddeel CYP3A4 induceert in het darmslijmvlies.180,264 Verschillende onderzoeken hebben de impact van andere knoflookformuleringen op CYP3A4-substraten geëvalueerd en hebben geen effect gevonden. De meeste producten in deze onderzoeken bevatten weinig of geen allicine.182,178,263,265

Saquinavir (Fortovase, Invirase): knoflook dat allicine bevat zou de saquinavirspiegels kunnen verlagen. Saquinavir is een substraat van cytochroom P450 3A4 (CYP3A4) iso-enzymen. Er bestaat bezorgdheid dat knoflookproducten die allicine bevatten CYP3A4 in de darm kunnen induceren en subtherapeutische niveaus van saquinavir kunnen veroorzaken. Dit is voornamelijk gebaseerd op een farmacokinetische studie die aantoont dat het tweemaal daags gedurende 3 dagen innemen van een specifiek knoflookproduct (GarliPure Maximum Allicin Formula, Natrol Inc.) de saquinavirspiegels met ongeveer 50% verlaagt. Er wordt gespeculeerd dat het allicinebestanddeel CYP3A4 induceert in het darmslijmvlies.180,264 Verschillende farmacokinetische studies hebben de impact van andere knoflookformuleringen op CYP3A4-substraten geëvalueerd en hebben geen effect gevonden. De meeste producten in deze onderzoeken bevatten weinig of geen allicine.178,182,263,265 Tot er meer bekend is over deze mogelijke interactie, moet knoflook met allicine voorzichtig worden gebruikt bij patiënten die saquinavir gebruiken.

Sofosbuvir (Sovaldi): het nemen van knoflook met sofosbuvir zou de effectiviteit ervan kunnen verminderen. Dieronderzoek bij ratten toont aan dat het dagelijks oraal toedienen van verouderd knoflookextract gedurende 14 dagen gedurende 14 dagen de oppervlakte onder de concentratie-tijdcurve (AUC) na een enkelvoudige dosis sofosbuvir van 40 mg/kg met 36% verlaagt, de klaring verhoogt met 63% en verlaagt de plasmaconcentraties na 1 en 8 uur met respectievelijk 35% en 58%. Er wordt verondersteld dat deze interactie het gevolg is van inductie van intestinale P-glycoproteïne-expressie door knoflook.166

Tacrolimus: knoflook zou het tacrolimusgehalte kunnen verhogen. In één casusrapport ervoer een levertransplantatiepatiënt die tacrolimus gebruikte verhoogde tacrolimusspiegels en leverbeschadiging na inname van een specifiek knoflooksupplement (Garlicin Cardio, Nature’s Way) tot driemaal de door de fabrikant aanbevolen dosis gedurende 7 dagen. Er wordt gespeculeerd dat knoflook hepatisch cytochroom P450 3A4 (CYP3A4) remde, wat de plasmaspiegels van tacrolimus verhoogde.167

Warfarine (Coumadin): knoflook zou het risico op bloedingen met warfarine kunnen verhogen. Rauwe knoflook en verschillende knoflookextracten hebben antibloedplaatjesactiviteit en kunnen de protrombinetijd verhogen.7,155,157,159,160,253,254 Daarnaast is er een melding van twee patiënten die een toename van een eerder gestabiliseerde internationale genormaliseerde ratio (INR) ervoeren bij gelijktijdig gebruik van knoflook en warfarine.254,268 Dit rapport is later echter besproken vanwege beperkte klinische informatie. Andere klinische onderzoeken hebben geen effect van knoflook op de INR, de farmacokinetiek van warfarine of het risico op bloedingen aangetoond.269,270 Er is meer bewijs nodig om de veiligheid van het gebruik van knoflook met warfarine te bepalen.

Supplementen

Supplementen met anti-stollingseffecten: knoflook kan plaatjesaggregatieremmende effecten hebben. Het gebruik van knoflook met andere producten die het risico op bloedingen verhogen, kan additieve effecten hebben. Rauwe knoflook en verschillende knoflookextracten hebben antibloedplaatjesactiviteit en kunnen de protrombinetijd verhogen.7,155,157,159,160,253,254

Supplementen met hypoglykemische effecten: knoflook zou mogelijk hypoglycemische effecten kunnen hebben. Het gebruik van knoflook met andere producten met hypoglykemisch potentieel kan het risico op hypoglykemie vergroten. Knoflook en knoflookextract kunnen de bloedsuikerspiegel verlagen.22

Supplementen met hypotensieve effecten: knoflook zou mogelijk hypotensieve effecten kunnen hebben. Knoflook en knoflookextracten hebben bloeddrukverlagende effecten.225,231,236,255,256,257,261,271,272 Theoretisch gezien zou het combineren van knoflook met andere kruiden of supplementen met hypotensieve effecten het risico op hypotensie kunnen verhogen.

Aandoeningen

Stollingsstoornissen: knoflook zou het risico op bloedingen kunnen verhogen. Rauwe knoflook en verschillende knoflookextracten hebben antibloedplaatjesactiviteit en kunnen de protrombinetijd verhogen.7,155,157,159,160,253,254

Preoperatief: knoflook heeft antibloedplaatjes- en hypoglykemische effecten, die bij perioperatief gebruik overmatig bloeden kunnen veroorzaken of de bloedglucoseregulatie kunnen verstoren. Ten minste één casusrapport suggereert dat het consumeren van ongeveer 4 gram geplette knoflook bij elke maaltijd gedurende een paar weken, en 12 gram knoflookpoeder de avond voor de operatie, de protrombinetijd verhoogde en bloeding verlengde.273
