Wat is saffraan?

Saffraan is een specerij, kleurstof en geneesmiddel afkomstig van de gedroogde stamper van de plant Crocus sativus. Saffraan wordt al zo’n drieduizend jaar gebruikt, met name in landen als India, Griekenland, Frankrijk, Spanje, Italië en ook Iran, waar de meeste saffraan ter wereld wordt verbouwd.1,2,3,4,5

Wat is saffraan?

Gebruik

Het dagelijkse leven kent talloze impulsen die chagrijnig, prikkelbaar of nukkig kunnen maken. De oorzaak van een verminderde gemoedstoestand vinden we normaliter in een afname van de serotonine en dopamine aanmaak in de hersenen.6,7 Dit zijn namelijk de neurotransmitters die samenhangen met stemming. Uit onderzoek blijkt dat saffraan zorgt voor een toename van de stemmingsgerelateerde neurotransmitters in het brein.8,9,10 Deze bevindingen vertalen zich ook in wetenschappelijke onderzoeken gerelateerd aan de stemming. Zo bleek in verschillende studies dat saffraan een significant positief effect had op het gemoed.10,11,12

Hierdoor wordt saffraan hoofdzakelijk ingezet bij depressie en de ziekte van alzheimer. Meta-analyses en klinische onderzoeken naar volwassenen met een milde tot matige depressieve stoornis tonen aan dat het nemen van saffraanextract of gedroogde saffraan gedurende 6-12 weken de symptomen van depressie verbetert.1,12,13,14,15,16,17,18 Ook bij alzheimer lijkt saffraan een bevorderlijk effect te hebben. Zo blijkt uit enkele kleine, klinische onderzoeken dat de inname van saffraanextract bij alzheimer eenzelfde positief effect heeft op het cognitieve vermogen als meer gangbare medicijnen.19,20,21

Gebruik

Werking

De delen van saffraan die in de traditionele geneeskunde worden gebruikt zijn het bovenste deel van de stamper, ook wel het stigma genoemd, en de bloembladen. Het stigma wordt doorgaans vaker gebruikt dan de bladen. Saffraankruid komt eveneens van het stigma.

De belangrijkste bestanddelen van saffraanstigma zijn crocine, picrocrocine en safranal.22,23,24,25,26 De crocinen zijn chemische verbindingen uit de carotenoïden, die saffraan zijn karakteristieke geelrode kleur geven.22,27 Alfa-crocine is het meest voorkomende bestanddeel; echter, crocetine lijkt het meest verantwoordelijk te zijn voor de werking van saffraan.28 Het stigma bevat ongeveer 4% picrocrocine, dat verantwoordelijk is voor de bittere smaak van saffraan.28 Safranal maakt ongeveer 70% uit van de etherische olie en geeft saffraan zijn aroma.22,27,29

Andere bestanddelen van saffraan zijn de fenolzuren, zoals looizuur, galluszuur, cafeïnezuur, kaneelzuur, chlorogeenzuur, ferulazuur en vanillinezuur.30 De bestanddelen van het saffraanstigma ondergaan tijdens de ontwikkeling kwantitatieve veranderingen.31 Opslag- en droogtemperaturen lijken de hoeveelheden van sommige bestanddelen te beïnvloeden; echter heeft vriesdrogen geen negatief effect op de hoeveelheid safranal.22,32,33

Anti-astmatische effecten

Uit in vivo onderzoek blijkt dat saffraan de astmasymptomen die verband houden met een allergische reactie in bescheiden mate verbetert. Hoewel het werkingsmechanisme niet precies duidelijk is, heeft het kruid mogelijk een antioxiderende, ontstekingsremmende, immunomodulerende en spierverslappende werking, zoals voornamelijk aangetoond in diermodellen.26

Antistollende effecten

Laboratoriumonderzoek heeft laten zien dat sommige bestanddelen van saffraan de bloedingstijd en de bloedplaatjesaggregatie kunnen beïnvloeden. Echter blijkt uit klinisch onderzoek dat het nemen van 200 mg of 400 mg saffraan per dag gedurende 7 dagen geen invloed heeft op de bloedtijd of de niveaus van fibrinogeen, eiwitten C en S, of factor VII in vergelijking met een placebo.47,48,49 Het gebrek aan effect kan te maken hebben met de aanwezigheid van bestanddelen in saffraan die zowel de bloedplaatjesaggregatie induceren als remmen.47

Antidepressieve effecten

Saffraan wordt van oudsher gebruikt bij de behandeling van depressie en heeft positieve effecten laten zien in klinisch onderzoek.1,3,12,13,14,15,50,51 Van saffraanextract is ook aangetoond dat het een negatieve stemming, angst en stress verbetert in vergelijking met het placebo.52 Er is ook enige interesse in de vraag of saffraan zou kunnen helpen om hunkeren naar en symptomen van opioïdontwenning te verminderen.53,54 In dieronderzoek werd gedacht dat de antidepressieve effecten van saffraan te wijten waren aan crocine; andere bestanddelen kunnen echter niet worden uitgesloten.1,55 Er is gesuggereerd dat saffraan de niveaus van serotonine in de hersenen zou kunnen moduleren door de heropname van serotonine te remmen evenals de niveaus van dopamine en noradrenaline te veranderen.1,3,15,54 Er is echter geen bewijs dat dit mechanisme ondersteunt.

Antidiabetische effecten

Klinisch onderzoek suggereert dat het nemen van saffraanextract de toename van de nuchtere bloedsuikerspiegel vermindert wat vaak wordt gezien bij antipsychotica.56 Ander klinisch onderzoek suggereert dat saffraan, saffraanextract en crocine de bloedsuikerspiegel kunnen verlagen. De resultaten bij mensen zijn echter inconsistent.5,57 Onderzoek in diermodellen suggereert dat de hypoglycemische effecten van saffraan te wijten zijn aan verbeteringen in de insulinesecretie en vermindering van de insulineresistentie.58,59

Anti-epileptische effecten

Saffraan wordt traditioneel gebruikt voor epileptische aanvallen. In een diermodel verminderde het component safranal de aanvallen.60,61,62

Anxiolytische effecten

Saffraan wordt traditioneel gebruikt bij de behandeling van angst. Voorlopig klinisch onderzoek toont aan dat het nemen van saffraan de symptomen verbetert bij patiënten met lichte tot matige angst.15 In dieronderzoek werden de angst reducerende effecten van saffraan in verband gebracht met de bestanddelen safranal en crocine.63,64

Cardiovasculaire effecten

Voorlopig bewijs suggereert dat crocetine invloed kan hebben op de cardiovasculaire activiteit.70 Er zijn aanwijzingen dat het een rol speelt bij het verlagen van het cholesterol- en triglyceridengehalte.27,65 Bovendien bindt crocetine zich aan albumine, wat mogelijk de zuurstofdiffusie verhoogt en aderverkalking verbetert.66 Antioxiderende effecten van crocetine kunnen ook een rol spelen bij het verminderen van de oxidatie van lipoproteïne met lage dichtheid (LDL)-cholesterol.67,68 De antioxiderende effecten van crocetine kunnen ook de vasculaire gladde spiercellen beschermen tegen proliferatie.59

Chemoprotectieve effecten

Saffraan kan beschermen tegen toxiciteit van chemotherapeutische middelen. In dieronderzoek verlengde saffraanextract, alleen of in combinatie met andere antioxidanten, de levensduur en voorkwam bepaalde vormen van lever- en niertoxiciteit, evenals afname van het lichaamsgewicht, hemoglobinegehalte en leukocytenaantallen.71,72,73 Van crocetine, een bestanddeel van saffraan, is aangetoond dat het beschermt tegen blaastoxiciteit bij muizen.74

Beschermende effecten tegen erectiestoornissen:

Er is tegenstrijdig bewijs bij mensen met betrekking tot het gebruik van saffraan of de bestanddelen ervan voor erectiestoornissen.2,75,76 In dieronderzoek verhoogt het bestanddeel crocine de frequentie van erecties en verkort het de tijd tot ejaculatie, terwijl safranal deze effecten niet vertoont.9 Van het bestanddeel crocetine wordt gedacht dat het de productie van vasculair stikstofmonoxide verbetert, een sleutelfactor tijdens erectie.76

Sportgerelateerde effecten

Van saffraan wordt gedacht dat het de maximale zuurstofconsumptie tijdens inspanning verhoogt, wat resulteert in een langer uithoudingsvermogen. Er wordt verondersteld dat dit verband houdt met een verhoogd spiermetabolisme en/of neuronale functie.77 In menselijk onderzoek lijkt saffraanextract te beschermen tegen spierbeschadiging, waardoor de niveaus van creatinekinase en lactaatdehydrogenase worden verlaagd.78

Gastro-intestinale effecten

Het is aangetoond dat kleine hoeveelheden saffraan de maagafscheiding stimuleren.79

Hypolipidemische effecten

Het is onduidelijk of saffraan hypolipidemische effecten heeft. In de meeste onderzoeken bij mensen verlaagt saffraanextract het cholesterolgehalte niet.56,81 Zowel saffraan als het bestanddeel crocine hebben echter hypolipidemische effecten aangetoond in diermodellen.82,83

Hypotensieve effecten

Onderzoek bij dieren en sommige mensen suggereert dat saffraan de bloeddruk kan verlagen.2,84,85,86 Crocine en safranal kunnen op zijn minst gedeeltelijk verantwoordelijk zijn voor dit effect.84

Immunomodulerende effecten

Van saffraan wordt gedacht dat het immunomodulerende effecten heeft. In laboratoriumonderzoek stimuleert saffraan de proliferatie van T-cellen.87 Echter, uit ander laboratoriumonderzoek blijkt dat het bestanddeel crocine de activering van T-cellen remt.88 In een diermodel voorkwam saffraan een auto-immuunreactie. Er is gesuggereerd dat de carotenoïde bestanddelen van saffraan immuuncellen zouden kunnen beschermen tegen oxidatieve schade. In menselijk onderzoek verhoogde saffraanextract de niveaus van IgG en verlaagde de niveaus van IgM.87 Dit suggereert dat saffraanbestanddelen immunomodulerende effecten kunnen hebben.

Geheugenverbeterende effecten

Saffraan wordt traditioneel gebruikt om het geheugen te verbeteren. Dieronderzoek suggereert dat saffraanextract het geheugenherstel zou kunnen verbeteren.63,89,90

Er is onderzocht of saffraan de symptomen van de ziekte van Alzheimer kan verminderen.20 In dieronderzoek verbetert crocine het leervermogen en het geheugen na consumptie van ethanol en beschermt het bepaalde receptoren in de hersenen, waarschijnlijk door de antioxiderende werking.20,91,92,93,94,95 Bestanddelen van saffraan bleken ook beschermende, antioxiderende effecten te hebben in dierlijke modellen.96,97 Sommige klinische onderzoeken tonen aan dat saffraanextract de symptomen van de ziekte van Alzheimer verbetert.19,20,98

Beschermende effecten voor de ogen

Saffraanextract is van belang voor de bescherming van de fotoreceptoren in de ogen tegen degeneratie, oftewel de achteruitgang. In dieronderzoek is aangetoond dat de bestanddelen crocine en crocetine de samenstelling en functie van de fotoreceptor beschermen tegen celdood.99,100,101

Tyrosinase-remmende effecten

Tyrosinase is een enzym dat de productie van pigmenten uit tyrosine katalyseert. Sommige saffraanbestanddelen hebben in laboratoriumonderzoek tyrosinaseremmende effecten aangetoond. Het gaat hierbij om kaempferol, geïsoleerd uit saffraanbloemblaadjes, evenals crocins en crousatins.102, 103

Werking

Veiligheid

Saffraan is volledig veilig om binnen te krijgen in de hoeveelheden die normaalgesproken ook in voedingsmiddelen zitten. Saffraan heeft in de VS de status ‘algemeen erkend als veilig’ (GRAS) voor gebruik als specerij of kleurstof voor levensmiddelen.104 Ook is saffraan veilig te gebruiken in grotere hoeveelheden tot 100 mg per dag, op korte termijn. Saffraanextracten zijn duidelijk veilig gebruikt in diverse klinische studies gedurende maximaal 26 weken.5,12,13,16,19,20,52,56,57,87,105,106,107,108,109,110 Het saffraanbestanddeel crocine is eveneens schijnbaar veilig gebruikt in een dosis van maximaal 30 mg per dag gedurende maximaal 3 maanden.3,53,81

Het innemen van saffraan in hoge doses is mogelijk gevaarlijk. Het nemen van 5 gram of meer saffraan kan ernstige bijwerkingen veroorzaken. Doses van 12-20 gram kunnen dodelijk zijn.79,111 Tijdens de zwangerschap is het mogelijk gevaarlijk om hoeveelheden in te nemen die groter zijn dan die gewoonlijk in voedingsmiddelen worden aangetroffen. Grotere hoeveelheden saffraan hebben baarmoederstimulerende en abortieve effecten: Vermijd in dit geval het gebruik van saffraan.79

Veiligheid

Interacties

Medicijnen

Het gelijktijdig gebruik van saffraan met geneesmiddelen tegen diabetes zou het risico op een te lage bloedsuikerspiegel kunnen verhogen. Sommige klinische onderzoeken tonen aan dat het nemen van saffraanextract de nuchtere glucosespiegels verlaagt wanneer het naast hypoglycemische middelen wordt gebruikt.5 Het is echter niet aangetoond dat saffraanpoeder zelf de nuchtere glucosespiegels verlaagt.57

Verder kan de combinatie van saffraan en antihypertensiva additieve effecten hebben. Dierlijk en menselijk onderzoek laat namelijk zien dat saffraanextract de bloeddruk kan verlagen.2,84,85,86 Ook zou het kruid het metabolisme van cafeïne kunnen remmen. Een kleine klinische studie suggereert dat het dagelijks innemen van 300 mg saffraanpoeder in 150 ml water gedurende 5 dagen en vervolgens het nemen van 200 mg cafeïne de cafeïnemetabolietspiegels in het speeksel en de urine bij mannen lijkt te verlagen, maar niet bij vrouwen. Theoretisch kan dit te wijten zijn aan de remming van cytochroom P450 1A2 door saffraan.4

Als laatste kan het gelijktijdig innemen van saffraan en middelen die het centrale zenuwstelsel onderdrukken een extra sedatief effect hebben. Klinisch onderzoek toont aan dat het dagelijks innemen van 60 mg saffraan-extract gedurende 26 weken slaperigheid en sedatie kan veroorzaken.107 Dieronderzoek suggereert dat het toevoegen van saffraan aan het geneesmiddel hexobarbital het slapen verder verhoogt en de motorische activiteit vertraagt.90

Kruiden en supplementen

Saffraanextract zou mogelijk de bloedglucosespiegels kunnen verlagen. Waar sommige klinische onderzoeken aantonen dat saffraan de bloedglucose verlaagt, laat ander onderzoek geen effecten zien.5,57 Daarnaast is er ook dierlijk en menselijk onderzoek dat suggereert dat saffraanextract de bloeddruk kan verlagen.2,84,85,86 Theoretisch gezien kan het gebruik van saffraan met andere hypoglycemische of hypotensieve supplementen additieve effecten hebben.

Tot slot kan saffraan kalmerende eigenschappen hebben. Saffraan heeft slaperigheid veroorzaakt in klinisch en dieronderzoek. Het gebruik van saffraan met andere kruiden en supplementen met kalmerende eigenschappen kan daarom mogelijk additieve effecten hebben.90,107

Dosering

Saffraanextract wordt meestal door volwassenen gebruikt in doses van 20-100 mg per dag gedurende maximaal 3 maanden. Grotere doses saffraanpoeder van 200-300 mg per dag zijn gedurende maximaal 10 dagen gebruikt. Het saffraanbestanddeel crocine wordt doorgaans gebruikt in doses van 15-30 mg per dag gedurende maximaal 12 weken.
